JBLE Form 1, Base Access Request Example and Instructions – Employment/Contractors

Per DoDM 5200.08v3 the Sponsor is responsible for ensuring a


valid reason for access and must be a DoD ID Card Holder. This
section will not list a contractor as the Sponsor based on policy.
PAGE 2 Instructions:
Blocks 1-13c must be completed including a signature on 13b (4 is
optional).
Block 8, 9, 10, and 12b are radio buttons and only one can be
selected. If you have US Citizen employees, a separate form
should be completed for Foreign Nationals and US Citizens as
different processes apply. This will speed process times.
Block 11 should describe specific reason, location, and cite the
contract number.
12a / 12b List the Prime and Subcontract as applicable. eVerify is
required for most contracts, select to confirm completed. The
eVerify documentation must be submitted for all visitor requests

PAGE 3 Instructions:
Provide the full name as it appears on immigration documents,
include all surnames, DOB as Date/Month/Year, Citizenship
should match the immigration documents, Valid ID types are listed
on Page 1, State/Country is the Issuing Location (US for EAD, i551),
ID number must be taken directly from the immigration
documents.

i551 (Green Card) – Not US Citizens and require Employment Authorization – Not US Citizens and
JBLE Form 1 to be marked Foreign National. Page 3 require JBLE Form 1 to be marked Foreign

Hours applies and includes business hours of AAFES, MWR, and
