Note: This form is to be completed in full by prospective contractors who are intending to undertake work for the Agent. The details in
this form will be used to verify the contractor’s details and to determine whether the contractor should be engaged to complete work
from time to time. The Terms and Conditions which follow this application form will apply to any work that the Agent engages the
Contractor to undertake. By signing this form, the Contractor agrees to the attached Terms and Conditions, which will apply each time
if, and when, the Contractor is engaged to provide services.
Should there be an inconsistency between the attached Terms and Conditions and any terms and conditions contained in any work
order received from the Agent, the terms and conditions contained in the work order will prevail.
We recommend that the Agent seeks independent legal advice in relation to this and the Terms and Conditions themselves.
