Note

1. Normal values for serum bile acids and transaminases may occasionally be seen, women with
persisting pruritus and normal bile acids / ALT should have repeat tests every 1-2 weeks
2. Results should be interpreted with caution in Ursodeoxycholic acid (UDCA) treated patient as UDCA
itself is a bile acid and may get measured by this assay
3. For patients treated with UDCA - LCMS/MS is recommended method for monitoring response to
treatment

Comments
Total bile acids are metabolized in the liver and can serve as a marker for normal liver function. Increases in
serum bile acids are seen in patients with Acute Hepatitis, Chronic Hepatitis, Liver Sclerosis, Liver Cancer,
and Intrahepatic Cholestasis of Pregnancy (ICP). ICP can lead to increased risk of fetal distress, preterm
delivery and even still birth. Itching of normal skin, liver dysfunction and elevated bile acid concentrations are
non-specific and have a wide range of causes. A healthcare professional should carry out a structured
history and examination, and consider other potential diagnoses: these may be pregnancy specific (including
pre-eclampsia) or coincidental to the pregnancy.

1. In an asymptomatic patient, Non alcoholic fatty liver disease (NAFLD) is the most common cause of
increased AST, ALT levels. NAFLD is considered as hepatic manifestation of metabolic syndrome.
2. In most type of liver disease, ALT activity is higher than that of AST; exception may be seen in Alcoholic
Hepatitis, Hepatic Cirrhosis, and Liver neoplasia. In a patient with Chronic liver disease, AST:ALT
ratio>1 is highly suggestive of advanced liver fibrosis.
3. In known cases of Chronic Liver disease due to Viral Hepatitis B & C, Alcoholic liver disease or NAFLD,
Enhanced liver fibrosis (ELF) test may be used to evaluate liver fibrosis.
4. In a patient with Chronic Liver disease, AFP and Des-gamma carboxyprothrombin (DCP)/PIVKA II can
be used to assess risk for development of Hepatocellular Carcinoma.
