Confined Space Entry Checklist
1■■ Pre-Entry Preparation
● ■ Confirm the space is a confined space (limited entry/exit, not designed for continuous occupancy)
● ■ Conduct hazard identification (toxic gases, oxygen levels, flammable vapors, engulfment risks)
● ■ Prepare Job Safety Analysis (JSA) / Risk Assessment
● ■ Obtain a valid Confined Space Entry Permit
● ■ Ensure permit is signed by competent authority
● ■ Post permit at entry location
2■■ Equipment Checklist
● ■ Calibrated multi-gas detector (O■, CO, H■S, LEL)
● ■ Continuous atmospheric monitoring device
● ■ Safety helmet with chin strap
● ■ Flame-resistant coverall or appropriate clothing
● ■ Safety shoes (antistatic)
● ■ Gloves (chemical / cut resistant as per job)
● ■ Safety goggles / face shield
● ■ Hearing protection (if noisy environment)
● ■ Respiratory protection (SCBA / airline respirator if required)
● ■ Fall protection (full-body harness + lifeline)
● ■ Tripod & winch (for vertical entry)
● ■ Rescue rope and retrieval system
● ■ First aid kit nearby
● ■ Two-way radio or communication device
● ■ Fire extinguisher (if flammable risk present)
3■■ Personnel Checklist
● ■ Entrants trained in confined space hazards and PPE use
● ■ Attendant (standby person) available at entry point
● ■ Entry Supervisor present to verify and authorize entry
● ■ Rescue team on standby, trained and equipped
4■■ Procedures Before Entry
● ■ Isolate the space (lockout/tagout electrical, mechanical, hydraulic sources)
● ■ Ventilate the space until atmosphere is within safe limits
● ■ Test atmosphere: Oxygen (19.5–23.5%), Flammable gas (<10% LEL), Toxic gases within permissible
exposure limit
● ■ Place signage “Confined Space Work in Progress – Authorized Entry Only”
● ■ Brief all workers on Emergency Evacuation Plan
● ■ Verify lighting is intrinsically safe (if required)
5■■ During Entry
● ■ Maintain continuous atmospheric monitoring
● ■ Ensure attendant remains outside & keeps count of entrants
● ■ Keep communication open between entrants and attendant
● ■ Monitor work progress, stop work if alarm triggers
● ■ Maintain good housekeeping (remove waste promptly)
6■■ Post-Entry / Closing
● ■ Ensure all personnel have exited safely
● ■ Close and sign off the permit-to-work
● ■ Remove lockout/tagout only after confirming space is clear
● ■ Store equipment properly and record entry in logbook
● ■ Conduct a debriefing to note any issues for future improvement
