Example: SAM P. LESTUDENT

Born October 19, 1994 in Hartford, Connecticut, I had an incurable case of Know-It-All Disease from day one. At the age of two, I was correcting my much older brother who called a silo a barn. I could read and write by three. At four, I started school and loved every second of it. Everything was easy for me, and I expected life to stay that way. I was quiet in class, but I picked things up quickly. I was even recommended for a summer program for People to People World Leadership in the summer after eighth grade, which I enjoyed participating in. Since entering high school, Ive learned that life isnt such a breeze. Just this year, Ive learned exactly how late some late nights can be if you procrastinate. Not only that, I got my first report card B. Ive been humbled by my experiences, but theyve taught me to work harder than ever.

High School Goal(s):

High School Goal Plan: In this space students should type a thorough paragraph describing their goals for the rest of high school for grades of 10-12. This may include extra-curricular activities, athletics, possible courses in a certain discipline, AP courses to take, etc. These goals may then include immediate goals such as attending a post-secondary school/university or serving in the military.

Life Plan Goal(s):

Life Goal Plan: In this space students should type a paragraph describing their long term goals, AFTER being graduated from Spring-Ford. These goals may include long term goals such as career and employment in a specific field or desire to live in a particular geographic location. Potential hobbies or interests. Students should also explain how they plan to reach these goals.

GIEP Goal(s) : USE 1 SLIDE PER GIEP GOAL

In this space students should provide their ninth grade GIEP goal (describe it, copy it from your GIEP, talk with parents about it) Discuss how this specific goal was reached or what progress you have made with this goal. Include specific activities, projects, actions, behaviors, etc. which serve as evidence for progress, growth, or achievement of this goal. You may NOT simply say that you worked on this goal and feel as if you accomplished it. If student feels the goal was not achieved or mastered, explain why and what you might do to develop it for the remainder of high school.

SAMPLE SLIDE: GIEP Goal-Self Advocacy

My second GIEP goal was to develop my self advocacy skills. I wanted to effectively communicate and assert my interests. I can at times be shy or hesitant to participate in class. To assure that I am understanding the curriculum, I need to ask more questions. I also need to be able to communicate with teachers when if I am struggling on a certain topic. I feel that I need to improve more on this goal. I feel uncomfortable when addressing teachers or asking for help. I always complete things independently even if I do not understand the assignment.

SAMPLE!!!
Science Squamous Cell Cancer Project- My partner and I made a great poster that had all of the accurate information necessary, but was also very appearing to the eye. A lot of classmates and my science teacher really liked it. Albinism Presentation- My partner and I made a power point that had a large supply of information that we were able to simplify from our research. Our presentation was also appealing with appropriate transitions and sounds for each topic. Spanish Latin American Artist Project- I wrote an entire page in Spanish describing my research on Salvador Dal. I was very proud that I was able to say so much in a second language and learned a lot of new words while completing the project. Postcard- I created a postcard from Puerto Rico. I had just traveled there last year and went there again shortly after the project so I was able describe tourist attractions in detail and was truly excited for what else I would do there. In addition, Im artistically challenged but was still able to make a pretty good picture of El Morro.

Vocational/Job Experience:

Provide a list of any job(s) you have outside of school Working somewhere? Coaching? Giving lessons? Babysitting? Camp Counselor?

Communication Skills- I am one of very few kids in all my classes that actually participates in class and communicates with teachers. Leadership Ability- Many of my friends look to me for advice or help with school work which shows they look up to me in some way. When they need help, I guide them in the right direction and away from bad choices. Outreach/Community Service- As I mentioned earlier, SADD took a trip to elementary schools and gave bullying presentations. Also, I plan on volunteering for the Towne Book Centers story time activity for kids. Creativity and Innovation- I have been interested in art all my life and have had many pieces in Spring-Ford art shows. Organization/Planning Ability- I have completed all long term assignments on time in my entire academic career.
