Align data migration tool docs with host/port mapping and JDBC timeouts

What I'm doing:

• Updated EN/JA/zh data migration tool docs to describe host/port mapping format and IP column matching for hosts.
• Added JDBC timeout parameters to configuration snippets and parameter tables in EN/JA/zh docs.
