fix bad alignment of song title and author

backport pulse width handling from atarisid7

It differs from 6.0 released at AtariAge that it includes an option to
NOT include all the 8580 mixed waveforms, which aren't needed for most
old SID files. So far, I only found Cybernoid 2 to need the mixforms.
