After a discussion with @p-talapova , we agreed that it makes sense to simply use
hadm_idforvisit_occurrence.visit_source_value. This PR replaces the previous approach which concatenatedhadm_id(when available) with other variables. The new approach will make joining tables via hospital admissin (hadm_id) cleaner and also makes it clear whatvisit_source_valuerepresents, regardless of the source table.
