I know that the "ellipsoidal vertical systems" are not in the standard. I got the idea from the 3D systems as wkt1 with the option --allow-ellipsoidal-height-as-vertical-crs.
With a derived vertical CRS a geoid model can be "simulated" locally (with "vertical offset" or "vertical offset and slope").
