This release addresses the reorganisation of the DUO hierarchy, and specifically closes #47, #79, #95, #59, #45, #54, #55 and #94.

This release includes changes agreed at the GA4GH Toronto meeting and is the version of DUO to be submitted for GA4GH product approval. This version is output of the GA4GH DU working group, part of the gA4GH Data Use and Researcher Identity (DURI) stream.

A release including minor fixes (typos), in preparation for extension to the original list of codes
