Extract files to directory

- Provide /nodir option in case someone doesn't want that
- Use index IDs instead of number for consistent file naming and
  potential cross-referencing
- Add Kaitai Struct format definition

Added build instructions and WEM filenames are now using four digits.

Initial implementation with original Delphi source code.
