Summary

Remove references to the deprecated WithAtlas option from migrate.md documentation.
The WithAtlas option was removed in 01e21a2 (v0.14.2) as Atlas became the default and only migration engine.

This PR:

  • Removes the "Atlas Integration" section that contained outdated WithAtlas(true) example code
  • Reorganizes Diff and Apply hooks documentation as subsections under "Migration Hooks"
  • Fixes heading levels (#### → ###) for proper document structure

Test plan

  • Verified documentation builds successfully with npm run build
