The Institutional Failure Index (2017-2018) was a research project documenting the economic costs of institutional failures across multiple domains. Key findings included:

• Incarceration: $378.96B annual US cost
• Traffic: $162.85B annual US cost
• Pollution: $4.86T global welfare loss
• Fraud: 5.22% of all spending
