small tall screen fixes for ? and t overlay
if you can test it on your phone it would help us that use small screen
this fixes the problem with ? and t menus including a crash
yes pad.addstr will even crash on a small screen
it should not harm big screen experience
todo read toc.ncx
I will add a real toc and scroll to toc position inside pages
current toc is only a disguised list of html files in the book
