docs: update README for v1.8.0–v2.0.0 features and hooks#98

Fill gaps accumulated across the last several releases: - Add Conflict radar, Enforcement hooks, Cross-mission intelligence, and Admiral synthesis subsections under How it works - Extend Mission artifacts to document JSON lifecycle artifacts and the .nelson/memory/ store (v2.0.0) - Warn manual installers that hooks and agent-team settings are only wired via the plugin path; they are not copied by a skill-only install - Fix stale paths — nelson-data.py now lives at skills/nelson/scripts/ (moved in v1.9.1) — and expand the full file tree to include hooks/, the split nelson_data_* modules, and nelson_conflict_radar.py https://claude.ai/code/session_01EhuPKubuJaJzQkPBhbwdJQ
