Adds the Phase 1 evidence ledger for POL-REV-001. Technical verification is recorded separately from commercial proof. The release remains BLOCKED until all 30 checks, payment/payable evidence, and attestations are satisfied. Hosted Actions are additionally blocked by the account-level GitHub billing lock.
