Stability, appearance and upgrade process improvements

- hardcoded white/grey in nav header because they don't work on ICS
- Moved RestoreStarter.start() from BootMonitor to final part of app startup (it needs a DB and breaks upgrades and new installs)
- Prevented crashing in AppUpgrade code; cant really make upgrade fail and it's been applied everywhere now and always fucks up
- Added isPending() to RestoreStarter
- upgraded all tools to latest
- removed some dead activities from manifest
