Add a Release notes page to the reference section covering v0.1.0 through v0.3.0, and bring the output formats page up to date with the markdown format, the color rules, and shrink-to-fit tables.

Adds a Release notes page to the reference docs covering v0.1.0 through v0.3.0, and brings the Output formats page up to date with the renderer work that shipped in v0.3.0.
reference/releases.md, linked from the reference index (weight 35, before troubleshooting).reference/output.mdnow documents the-o markdownformat (aliasmd) and pipe escaping, the color rules (--color,NO_COLOR, plain when piped), and shrink-to-fit tables.Hugo builds clean locally.
