Indications: Serious anaerobic infections especially those cause by Bacteroids fragilis.
Alternative to penicillin in some severe staphylococcal and streptococcal
infections, including staphylococcal osteomyelitis.
Contraindications: Should not be given to patients known as hypertensive or who have
experienced reactions with lincomycin.
Dosage: 300mg, three times daily
Mechanism of Action: Inhibits bacterial protein synthesis by binding to 50S subunit of
ribosome.
Adverse Reactions: Diarrhea, nausea, vomiting, abdominal cramps, abnormal taste.
Nursing Responsibilities:
Advice client to take the drug at the time prescribed by the physician.
Advice patient that drug is for external use only and to avoid contact with eyes.
Instruct patient to use medication only as prescribed.
Instruct client to take the full dose prescribed to help prevent resistant bacteria.
Instruct client that if diarrhea develops and continues for more than 24 hour, consult
physician promptly.

Indications: Management of RA, short-term symptomatic treatment of acute

treatment of ankylosing spondylitis, symptomatic relief painful and/ or other
inflammatory conditions.
Contraindication/ CI: hypersensitivity reactions to aspirin, meloxicam or other NSAIDs;
active or recurrent peptic ulcer; severe hepatic failure; nondialyzed severe renal failure;
gastrointestinal or cerebrovascular bleeding; other bleeding disorder.

Mechanism of Action:
Chemical effect: may be related to prostaglandin ( cyclooxygenase ) synthetase
inhibition.

Adverse Reaction:
GI discomfort, nausea, vomiting, hypersensitivity, anemias, lightheadedness, vertigo,
tinnitus, edema.
Nursing Responsibilities:
• tell patient to notify prescriber about history of allergic reactions to aspirin or
other NSAIDs before therapy
• oral solution may be substituted on a milligram - per - milligram basis for tablets.
Shaking suspension gently before use.
• tell patient to report signs and symptoms of GI ulcerations and bleeding, such as
vomiting blood, blood in tool, and black, tarry tools.
• instruct patient to report skin rash, weight gain, or edema.
• advise patient to report warning signs of hepatotoxicity (nausea, fatigue, lethargy,
pruritus, jaundice, right upper quadrant tenderness and flulike symptoms).
• warn patient with a history of asthma that it may recur during therapy and that he
should stop taking the drug and notify prescriber if it does.
• Inform patient that it may take several days before consistent pain relief is
achieved.

Indication:
Diet failed NIDDM. Adjuvant therapy in Type 1 Diabetis Mellitus patients who are not
well controlled with insulin.
Contraindication:
Contraindicated in patients hypersensitivity to the drug or any of its components; those
with hepatic disease , rental disease or dysfunction, or acute or chronic metabolic
acidosis; and in heart failure patients who needs other drugs. Temporarily without drugs
in patients undergoing radiologic studies involving parenteral administration of iodinated
contrast materials; using such products may result in acute renal dysfunction. if patient
enters hypoxic state, stop drug.
Dosage: 15mg, 1 tab, bid x 6 doses

Decreases sugar (glucose) production in the liver. It also increases sensitivity to the
body of insulin.

GI disturbances
Nursing Responsibilities:
• Tell patient to take once daily dose with breakfast ( except Fortamet or Glumetza,
which is to be taken with dinner ) and twice daily dose with breakfast and dinner.
• Tell patient not to crush or chew extended release tablets.
• Instruct patient to stop drug and tell prescriber about unexplained
hyperventilation, myalgia, malaise unusual somnolence, or other symptoms of early
lactic acidosis.
• Warn patient to avoid alcohol consumption while taking drug.
