ORDER
A verified petition for Cancellation and Correction of Entries in
the Certificate of Live Birth has been filed by Petitioner praying that
an Order/Decision be issued ordering defendants Office of the Civil
Registrar and the Philippine Statistics Authority to: (a) Admit the
affidavit of Acknowledgment and Admission of Paternity signed by
petitioner’s father, (b) Correct the name of the child in the Certificate
of Live Birth under Registry Number 00-1234; (c) Correct the middle
name of petitioner’s mother; and (d) Cancel the Certificate of Live
Birth in the year 2007 as Reg. No. 07-1123 for being duplicitous.

Finding the petition sufficient both in form and substance, let


the instant petition be set for hearing on May 28, 2019, at 8:30 in the
morning, at the Session Hall of this Court, at which date, time and
place, all persons who may have an interest in the petition are cited
to appear and show cause, if they have any, why the petition should
not be granted.

Petitioner is directed to cause the publication of this Order in a


newspaper of general circulation in the entire Province once a week
for three (3) consecutive weeks.
Let a copy of this Order be posted in three (3) conspicuous
places, to wit: Hall of Justice, Capitol; Municipal Hall; and Bulletin
Board of the Office of the Local Civil Registry.

Further, let a copy of the Petition and of this Order furnished


the Solicitor General, Makati City and the Local Civil Registrar..

SO ORDERED.
