1, what financial arguments could you use to justify your proposed recommendations to segregate FLTs

and the workers?

In current scenario you have purposed recommendation to segregate FLTs and workers with barrios and
designated walk ways of entrances both for pedestrian you have told that there is no money for that kind
of thing it will cause disruption to business now you have to convince them with financial arguments to
spend on safety rather than own accident because whenever accident happens it costs money the cost
of accident is many time higher then the cost of safety cost they have to face are direct in direct insured
uninsured as a result of an accident the direct cost involve first aid sick pay repair of FLT. damage to the
goods hospital treatment cost and loss of injured worker and rehabilitation cost of work (for six week to
recover) .The indirect cost company has to face worker absence and high staff turnover loss of staff
morale delay in fulfilling order , loss of face value and repetition investigation cost, remedial action cost
loss good will activation of penalties and the cost to comply with enforcement notice. It also incur
insured and uninsured cost insured cost will be like tip of ice berg it will include damage to FLT and
goods, compensation claim and employer liability insurance. Legal and medical cost uninsured loss can
cause severe damage to company. There are approximately ten times of insured cost and often referred
as uninsured loss ice berg. According to health and safety executive. They are 8-36 time higher than
insured cost uninsured incur sick pay, Criminal fines, investigation cost and time ,loss of face value and
repute, over time and training of employ, compensation not covered by insurance according to guidance
of heading of health and safety at work, benefits and cost that is cumulative cost of the accident is too
much higher than the cost of safety at work place Employers who spent on health and safety will get
several benefits of cost saving, reduce incidents and significant improvement in company productivity
and performance

2, (a) what unsafe behaviour would be included in the report?

• The FLT driver using mobile phone during driving which a severe violation result in accident

• The brakes were not applied at safe distance at safe distance rather than it was applied too late by
FLT driver

• Worker stepping into vehicle route in which FLTs are in operation

• No reporting system was for near miss and accident but in current scenario this is poorly unsafe
behaviour from workers and management
• No response of management on workhouse workers complain shows unsafe behaviour of
management.

• Using general work assessment which does not reflect the actual risk of workhouse and no one
highlighted that issue shows unsafe behaviours towards documentation assessment of risk and poor
implementation

• Reporting hazardous situation like an oil spillage is the responsible of employee but no one raised
the issue to rectify show unsafe behaviour towards hazardous situation

• Improper storage of stakes of goods in risk and blocking designated walkways by workers show
unsafe behaviours

• Poor housekeeping of stacked goods of oil spillage on vehicle route

• Management saying we don’t have money for such kind of thing shows UNSAFE management
behaviour towards safety.

• Lack of consultations committed between the workers and management because managing are
rarely seen in workhouse

• Must review the health and safety policy and procedure of the contractor company

• Establish cascade of roles & responsibilities of health & safety in organisation as there are no
health & safety representatives at present.

• Reporting policy &procedure will create positive reporting culture in the company.

• Legal compliance will increase and will reduce fines and enforcement actions

• Poor Incident/accident reporting, investigation and analysis-Many incidents go unreported.


Investigation only takes place after a serious accident. Analysis doesn’t consider human factors or go
beyond legal requirements.

• High absenteeism rate due to negative culture in company

• lack of workers consultation even manager was rarely seen in workplace and no records were
found for this particular aspect

• No or insufficient Risk assessment was not according to risk level and was found general

• No or insufficient training to the employees working in the premises of the project]

• Management blames individuals for accidents-Individuals are blamed, and it is believed that
accidents and injuries are a part of the job. Responsibility for accidents is seen as belonging to those
directly involved.

• Increased fines & enforcement actions due to legal breaches

• High staff turnover rate due to incidents &negative culture in company

• No Balance between safety and profitability-Profitability is the only concern in the organization.
Health and safety are seen as a cost, and the only priority is to avoid extra costs.

workers?

• Warehouse general hazards & risk level should be considered.

• Numbers of the workers that are working in the premises of the warehouse as the coverage must
be sufficient for all workers

• The nature of the work activities considered while deciding the suitable coverage of first aid.

• The shift pattern and the workers working in each shift must also considered

• The nature of workplace activities, equipment and environment must also be considered

• The history of accident, dangerous occurrences and incident must also be considered.

• The capacity and size of the organization warehouse must be in considered.


• The remoteness of the site from emergency medical services hospital

• The visitor’s, contractors and stakeholder of the assets must be kept in mind before arranging first
aid provision.

• The geographic location of warehouse section must also be considered.

• The coverage for the lone workers (venerable group) first aid provision must consider

• The first aid facilities must be easily accessible and the kit must be covering all basic first aid
equipment’s

• Must comply with local legislations regarding fir aid provisions

• According to ILO 1 first kit for 50 employees and one more for every fifty employees

representative must be taken in account as the workers leg was broken which requires urgent treatment
in hospital the following steps must be considered while investigation of accident

8) What individual human factors could have negatively influenced the behaviour of the young FLT
driver?

In present scenario the leg of a person is broken due to unsafe movement of vehicle asdriver so using
mobile phone and he is not attentive while driving so he brakes the vehicle late.

There is lack of risk perception so he is unable to notice the oil spillage on a ways of doing.

As the person are young drivers so there is lack of knowledge, ability, training and experience.
As the driver is using mobile so he skewing in his thinking .As driver is young so he may not have good
quality training and education of hazard risk of the job. While hiring company should keep that in notice
that driver should be capable and compenses.he should have proper knowledge. While driving in
warehouse. the driver should be given proper training before giving him a held of vehicle.

The attitude of a driving is not responsive towards job. As he is young and it takes time is develop a
attitude towards good health and safety. In this content workplace behavior plays an important role
because if could make the condition of incident worse if he would go on with such behavior.

There is lack of skills are developed with passage of time like physical skill in this content the driver of
fork lift truck does not own the skills so results in incident on site of warehouse results in injuries.

Motivation

There is lack of motivation as it is clearly seen nobody is reporting oil spillage on way of FLT.

Negative Culture

There is lack of positive culture in the warehouse as driver is using mobile phone while driving. It is
unprofessional behavior and the warehouse health and safety representative does not give important
developing the positive safety culture.

9) Based on the scenario only, what management failures could have contributed to this accident?

Under lying/ root causes are failure of management system. A key crucial the Safety and Hazard
Investigation is to pick the essential drivers of events, report the divulgences, and issue proposition to
shield for all intents and purposes indistinguishable scenes from happening. the insufficiency of
management structures that may have protected the scene from happening. there are lots of
management failures

· Poor management leadership and commitment was observed after observing the condition in the
workplace. There was no health and safety representation so all the HSE related operation were badly
affected.

· Safe system of work not available for warehouse work activities

· Lack of supervision by the management was observed as they were not practicing and sort of
active monitoring on the work site

· No incidents reporting policy & procedure available in company and that’s why no one is paying
attention towards near misses & accidents

· No record available for investigation and to reach on lessen learnt by rectifying root causes

· Lack of training to FLT drivers and other employees lead them to work as a lay man

· Periodic maintenance system for oil Spillage was not available in the warehouse area.

· Poor housekeeping system is clear indication of failure of management system

· Lack of consultation among the management and workers were observed and there were not
records of workers compliant

· Senior management was not discussing and conducting any sort of meeting regarding HSE issues of
their organization.

· Management was failed to develop suitable and sufficient risk assessment as it was inappropriate
and the risks and hazards associates for the warehouse and vehicle movement were not covered
precisely.

· Lack of designated pathways for vehicles (FLT) & pedestrian, barriers and crossing points in the
warehouse are management failure to provide safe place of work
· HAZCOM & HAZID system are not in place in organization

· Insufficient first aid provision is also gap in management system

· The is no proper storage area and stacking rules develop and implement by management

10) (a) Based on evidence from the scenario only, comment on how the organisation is performing in the
following areas.

(i) active and reactive monitoring?

• organisation was failed to comply active monitoring as there were no record of safety sampling, safety
tours, safety campaigns, awareness sessions, safety inspection and safety surveys.

• the risk assessment was insufficient and not suitable&inadequate and need be update after formal
enforcement action by inspector. incident investigation process after the FLT and pedestrian collision and
first aid policy was revamped, all are reactive monitoring.

(ii) the adequacy of current risk assessments?

• The current risk assessment developed by the organisation was found unsuitable and insufficient as it
is too general in nature and not reflecting actual risk in warehouse

• There were no records of the review of the risk assessment in any scenario. The current risk
assessment needs to be updated as per precise & actual risk level in warehouse to be effective

(iii) Operational procedures and rules?


The operational procedure & rules observed in the current scenario are

· It was observed that there was no appropriate policy, procedures, rules & SOP’sprovided by the
company regarding reporting & recording of incidents.

· It was also observed that there were no designated pathways, crossing point and separate
entrances for vehicles & pedestrian provided by company

(iv) Compliance with legal requirements?

• As per ILO (C155-R164 article 16, 10) employer must provide safe system of work, safe workplace,
equipment and environment. They were showing poor commitment towards health and safety.

10 (b) Based on evidence from the scenario only, what sources of benchmarking information could be
used?

The following information will be considered as sources of benchmark

· Warehouse Risk assessment must be taken into the account

· Near miss, dangerous occurrence can be benchmarked

· Staff turnover &Absentees rate is also a key factor to be considered


· Training records for the staff members at all levels must be examined

· Inspection and maintenance records must also be considered

· Worker complaint record

· Housekeeping record can be used

· Safety meeting minutes record and hazard communication at all levels
