1. Outline of breakdown
While sailing the Pacific Ocean in M0 operation, ‘Auto slowdown’ occurred due to excessive devia-
tion of the main engine exhaust temperature. After stopping the main engine and carrying out an in-
vestigation, the No. 2 FO pump roller guide had deviated about 15 degrees in the clockwise direction,
and about 150mm (2/3) of the roller (width 62mm) was broken in the circumferential direction. Fur-
thermore, slight damage had occurred to the fuel cam.
2. Causes
Due to roller guide safety pin breakage, the roller guide rotated, and the broken tip of the safety pin
chewed into the roller guide and stuck. Furthermore, damage of the roller and cam are surmised that
due to the above damages the roller and cam could not achieve smooth rotation and due to chewing
broken tip.
3. Repair
As emergency measures, the roller guide was completely replaced and a safety pin manufactured on-
board was attached and tried out. However, the pin manufactured on-board was not sufficient in terms
of strength. Thus, voyage was resumed with one cylinder cut off. After acquiring a spare, the pin was
replaced and repairs were made. Other cylinder safety pins were inspected as well.
