Breakdown order

Inform maintenance Technician/ foreman/ Engineer/ Manager whenever a breakdown of equipment occurs,
detailing the problem encountered, time, possible causes etc.

The maintenance Foreman/Engineer will identify the problem along with the Equipment user, and organize
the required resources and assign the job to technician who will execute the task.
Foreman/Engineer will inspect the performed task on equipment and hand over it to end user.
Foremen/Engineer generate breakdown order in SAP, update the details like cause, remedial action taken,
future preventive action plan, manpower confirmation, downtime details(if applicable) etc.

The equipment breakdown will be evaluated and the spare parts availability will be verified. If it is not
available in stock, the purchase requisition will be raised through SAP. Meanwhile possible rectification of
the problem will be done and give feedback to Department manager

Priority of Breakdown order is based on the following criteria

1.Very High
 Higher safety risk like human fatal
 Affecting Environmental norms, etc.
 Breakdown duration more than 24 Hrs.
Ex: Fire pumps, any equipment failure related to FGD operation, High Hazardous chemical spill over

2. High
 Higher production loss
 Higher operation and maintenance cost
 Major cause to damage the other related equipment /parts
 Breakdown duration more than 4 Hrs. and less than 24 Hrs.
Ex: Any paper machine rolls failure, Turbine Stoppage

3.Medium
 Minor production loss
 Minor operation and maintenance cost,
 Minor damage of other related equipment /parts
 Breakdown duration less than 4 Hrs.
Ex: Hood Exhaust fan, Hood Door system failures, Doctor Oscillation failure
