As parent/legal guardian of ________________________________________________, I hereby, give my


consent to send my son/daughter to the education trip my son/daughter is well aware of the preventive measures
and the precautionary measures in not coming in contact with the virus and in not spreading the COVID-19 to
any of the co-students/teachers.

I grant permission for him/her to participate in the fieldtrip described below.

Destination: Shimla & Manali via Delhi & Chandigarh

Date of leaving: 6th April 2021

Date of returning: 15th April 2021

I am aware that when I am on a college field trip, I am under the jurisdiction and supervision of the
college-employed agents and that my behavior must conform to the Code of Student Conduct, the college’s
reasonable instructions from chaperones. I understand I will be subject to appropriate disciplinary action for
violations of these rules and regulations. I also submit my consent that neither I nor any of my family members
have been infected/availed treatment for COVID-19 at the time of joining the trip. If I fell sick and I suffer from
COVID-19 it is my personal responsibility.
