CERTIFICATE OF STRUCTURAL INTEGRITY

This is to certify that the PROPOSED ROOF DECK ENCLOSURE (Additional Floor) OF THE

EXISTING TWO (2) STOREY RESIDENCE WITH ONE (1) BASEMENT AND ROOF DECK, owned by

the SPS. SEVERINO and ROMELDA BANGASAN located at No. 37 CAMDAS SUBDIVISION,

BAGUIO CITY, BENGUET is structurally safe and sound and its stability will not endanger life and

adjacent properties with its use for RESIDENTIAL OCCUPANCY only.

The as-built structural framing of THE EXISTING TWO (2) STOREY RESIDENCE WITH ONE (1)

BASEMENT AND ROOF DECK and the PROPOSED EXTENSION (ROOF DECK ENCLOSURE) will

withstand the Gravity Dead Loads and Live loads generated by its use/occupancy and the

Seismic Loads generated by its location (Zone 4) under Seismic Source Type-A as shown in the

attached structural frame analysis.

This Certification is issued based on the structural assessments conducted on site, the as-built

conditions and review of the As-Built Structural Plan Details provided by the Professional-on-

Record and the Professional-In-Charge of the Construction.
