AFFIDAVIT OF CONSENT

I, DARIO S. SALVATERO of legal age, married, Filipino with residence and postal address
at MANGANDINGAY, CABARROGUIS, QUIRINO. After having been duly sworn to in
accordance with law hereby depose and say:

1. That I am the legitimate parent of MARC DARIUS C. SALVATERO who was born on
september 5,1998 at MANGANDINGAY, CABARROGUIS, QUIRINO.
2. That, he is a Bachelor of Science in Accountancy student of Isabela State University.
3. That I allow my said son to undergo On-the-Job Training particularly in BUREAU
OF INTERNAL REVENUE DISTRICT OFFICE NO. 15, NAGUILIAN, ISABELA
for period of at least two (2) months or its equivalent in 200 hours of training work as
a continuing education and training development and career advancement.
4. That, I am freely giving my consent to my son to be on off-campus training
particularly in BUREAU OF INTERNAL REVENUE DISTRICT OFFICE NO.
15,NAGUILIAN, ISABELA and hereby waiving any claims accruing as a result of
negligent acts, errors, damages or breach or alleged breach during the said On-the-Job
Training to Isabela State University.
5. That you may treat him as your employee/worker but he is not entitled of any
compensation during the duration of the training.
6. That, I am executing this affidavit to attest to the truth and veracity of the foregoing
facts and statement, and permitting him to abide the rules and regulations imposed by
the school and the company.
