AFFIDAVIT OF UNDERTAKING WITH WAIVER

I, Jerald Malobo, Filipino, of legal age, a student of Fullbright College, and a resident of
Poblacion, San Vicente, Palawan, after having been duly sworn to a in accordance with law, hereby
depose and state.

1. That I shall have an On-the-Job training (OJT) outside of the premises of FULLBRIGHT
COLLEGE for a period to cover 270 hours which is an academic requirement for graduation
of the course Bachelor of Science in Criminology;
2. That I am willing/ready to comply with all the assigned tasks given to me with care and
diligence as part of my On-the-Job Training outside of the premises of FULLBRIGHT
COLLEGE;
3. Pursuant thereto, I undertake to act with extraordinary amount of care and diligence during
the OJT which shall be outside of the premises of FULLBRIGHT COLLEGE and hereby
waive my right to any injury or damage that may be suffered by the outside of the school’s
premises, as a consequence of my OJT;
4. Hence, FULLBRIGHT COLLEGE shall not be liable in any way to me, including its
personnel or officers during the period of my OJT;
5. That I take this OJT at my own risk knowing fully well the consequences of this WAIVER I
have made in a manner absolute and irrevocable in favor of FULLBRIGHT COLLEGE and
its officer and personnel;
6. That I am executing this affidavit of waiver to attest to the truth of the foregoing facts and to
comply with the requirements of the law and abide by all the rules and regulations of the
company where I will be undergoing my OJT.

IN WITNESS WHEREOF, I have signed the affidavit this 20th day of January 2025 at the City
of Puerto Princesa.
