III. PROBLEM PRESENTED
The client expresses that she is in need of medical assistance. She suffers in
various health problem.
IV. BACKGROUND INFORMATION
A. The Client
The client is known as “Babo Olay” in their neighborhood. She is kind and caring
mother to her family. She taking care of her mother all day. Her sisters assigned her
to do the routine. The client never attended in school due to poverty. The client is a
widow. She do the house chores while taking care to their mother. She also
sometimes helping her daughter in law in taking care of her grandchild. She did not
engage in any work outside. The client is experiencing many health problem. She
experienced cough headache, foot ache and numbness of her legs often.
The client cannot walk and work when she felt those difficulties. She was medically
check-up and take 3 classes of medicine for just two weeks a year ago. The client
did not take any medicine for her illness. Her sisters was worried to her condition.
The client stated that the prescription given by the doctor was loss. The client do
not know what kind of medicine she can take. The client has a Senior Citizen ID but
she do not know how to use it. The client and her sister helping each other in doing
house chores while the twin sister work. The client sisters are the provider of the
family.
They have a small sari-sari store before but when they experience scarcity their
capital was being used. The client stated that their sari-sari store before is a big help
to support their earnings. As stated by the client they have sewing machine that they
used to sew the clothes of walk-in costumer. Aside from the income of her sisters in
teaching Arabic lesson, they also gain sufficient income. Their family do not expense
too much every day. Their earnings is suffice to provide their daily needs.
B. THE FAMILY
The client’s family never attended school resulted that they cannot able to read and
write their name properly. However, the client and her sisters has a high educational
attainment in Arabic. The client’s twin sister is an Arabic teacher. They gains 1,000-
1,500 per month. The client twin sister also has a skill in sewing. Sometimes they
sewed the clothes of their walk-in customers were it serves as additional income.
Her son share his earnings to buy their needs. He works as driver and gains 200-
250 per day. His wife finished elementary level. Her daughter in law is a plain house
wife and did not venture in any business and work. She is taking care of their
children all day. The client and her family helped each other to survive their daily
living with a strong faith.
The client’s family live in Pansacala Street, RH-10 Cotabato City. Their house is
small for them. It is made of semi-concrete materials and has a cement post and
floor. And the walls are made of woods. The client’s house has only 1 room but
divided by two bed. The client and her family help each other in every circumstances
and do not seek help to their neighborhood. Their neighborhood is engaged in
betting and alcoholism so that even they wanted to Da’wah (teach) the good
standard behavior of Islam, the client’s family prefer to become deaf and blind just
not to get involved in trouble. They prefer to handle their own problem than asking
help to their neighbor. The client owned the house but the land was not directly
owned by them.
The client’s neighborhood called them “mga balo” because they are all widowed.
The client did not asked her sisters for medicine. She enclosed that she don’t want
to make her sister worry about her difficulties.
C. THE ENVIRONMENT
The place where the client live is Narrow Street. The houses in their community is
swampy. There prone to flood when the rain is heavy. The client and her family help
each other in every circumstances and do not seek help to their neighborhood. Their
neighborhood is engaged in betting and alcoholism so that even they wanted to
Da’wah (teach) the good standard behavior of Islam, the client’s family prefer to
become deaf and blind just not to get involved in trouble. They prefer to handle their
own problem than asking help to their neighbor. The client owned the house but the
land was not directly owned by them.
The client has senior citizen ID. She was also assisted before by a social work intern
in availing Senior Citizen ID. Her mother who reached a hundred years was also
applied in centenarian at Kabuntalan, North Cotabato. The client is active in the
activities of the barangay. She is always conscious about the announcement of their
barangay officials.
V. ASSESSMENT STATEMENT
The client is a senior citizen. At her age, she experienced various health difficulties.
She was medically checked 1 year ago. After a month of taking medicine, she
stopped her medication. The prescription of her doctor got lost and she did not go
back again to the doctor. She cannot easily go outside without assistance because
of her weak body. However, she wanted to go to the doctor for checking her health
condition with the help of others. So, the client will be link to City Health for availing
free medical checkup and free medicines.
The client’s sister is also a senior citizen. She is in need of assistance in complying
the requirements for Senior Citizen ID. Due to her weak body and illiterate she
cannot go outside and talk with other people fluently and independently. She is
ashamed and scared to mingle with new people. However, she wanted to comply
her requirements with an assistance. Despite of her situation she will be assisted in
availing Senior Citizen Id to Office of Senior Citizen Affairs through complying the
requirements. She will be assisted in having Barangay Certificate, Voters ID, Cedula
and 2X2 picture. The client and her sister will be assisted also in having booklet as
part of requirement in availing their discounts in medicines and groceries.
Access to these various services will be a big help to the client and her family. The
client will be medically checked and diagnosed if what is really the condition of her
health. She will get a free medicine in curing or reducing her health problems. Her
sister will be able to have a senior citizen ID to use enjoy the incentives and benefits
of the elders. The client and her sister will have a booklet to use in availing the
discount to their medicine and groceries. This will help the client in their daily needs
and live long well.
VI. PLAN OF INTERVENTION

curing her illness.
