1.0 Company Safety and Health Policy
It is the general policy of ___________________to perform work in the safest manner
possible consistent with good construction practices.
To achieve these objectives, we are committed to the following:
Identification of potential hazards that may arise from the
programmed/scheduled work activities;
Organizing the work activities so as to minimize the risk arising from them;
Ensuring that all workers are given orientation/briefing or induction prior to
deployment to the site;
Establishing a site safety committee to act as policy making body of the project
on all issues pertaining to safety and health;
Ensure the conduct of daily toolbox meetings;

3.0 Composition of Construction Safety and Health Committee (CSHC).
I ____Gilbert Rios_____ is committed to organize a CSH Committee at the project site to
act as a policy making body of the project on all issues pertaining to safety and health.
3.1 Composition of Construction Safety and Health Committee (SHC)

3.2 Functions of CSH Committee shall include but are not limited to the following:
Plans, develops and oversees the accident prevention programs for the
construction project;
Gather feedback, discuss, review safety policies, procedures and
recommend improvements;
Conducts safety/toolbox meetings.
Communicate safety and health concerns and information to management
and staff personnel;
Reviews reports of accident investigation and implementation of program;
Solicit, evaluate and confirm implementation of safety and health
suggestions from employees.

5.0 Duties and Responsibilities
a. Owner
Responsible in operation, safety and health of the entire project.
Delegate personnel for the implementation of safety
Chairman of the safety and health committee. Arranges schedule and place of
meeting and notifies members.
b. Project Coordinator
Ensures that all workers under his supervision understand safety responsibility.
Ensures that proper use of PPE
Acts without delay in addressing all hazards and unsafe conditions or actions
within the scope of their area of responsibility.
Holds toolbox safety meeting with the workers.
c. Worker
Consistently observes work conditions, operation, equipment and tools for the
purpose of accident prevention.
Complies with all the regulations as well as safe work practices and safety rules.
Uses all necessary personal protective safety equipment and other safe working
practices.
Corrects any unsafe practices within the scope of work operations and reports
any unsafe conditions.
Attend all committee/toolbox meetings.
6.0 Conduct of Toolbox Meeting
To hold a daily safety toolbox meeting every morning before the start of work to attain
the following objectives:
To discuss matters pertaining to the job;
To provide briefing on the planned work;
To predict potential hazards or actual occurrence of accidents at the site;
To provide tips and suggestions on how to prevent possible accidents, and;
To discuss other pertinent matters (e.g. accident prevention)
7.0 Personal Protective Equipment
The company will provide the following basic PPEs and enforce the use of the
adequate and approved type of personal protective clothing and equipment suited to the risk
inherent in the work and operations performed:

Safety shoes - will be used for general works and to be worn at all times on the
site.
Safety helmet – all workers must wear an approved safety helmet while on the
construction site.
Safety gloves - to be worn wherever possible for hand protection
Safety goggles/Face shield – protection against flying objects from doing work
such as chipping, grinding, cutting, drilling works and alike.
Safety belt/harness – to be used whenever working at height.

8.0 First-Aid, Health Care Medicines and Equipment Facilities
The owner/employer will provide the following basic first-aid kit and health care
medicines and facilities for workers in the site:

9.0 Workers Welfare Facilities
The following welfare facilities will be provided in the site to ensure humane working
conditions:
a. Adequate supply of safe drinking water
b. Adequate sanitary, washing and sleeping facilities separate for men and
women workers
c. Adequate facilities for changing and for the storage and drying of work
clothes.
d. Adequate accommodation for taking meals.

11.0 Construction Waste Disposal
The main constructor including subcontractors will be responsible for minimizing
waste generated during the implementation of the project. The following procedures for
disposal of wastes will be implemented in the site:
1. Ensure that the construction wastes are segregated from that of domestic
waste.
2. All domestic wastes are to be collected on a daily basis.
3. Construction debris (broken hollow blocks, spoiled concrete, loose
concrete, etc) should be taken out on the staging area.
4. Oil spills and spoiled greases should be wrapping in the black garbage bag
and will be properly disposed.
5. Application of good housekeeping.
12.0 Safety Work Practices - (indicate safety work practices based on the scope of work to be undertaken.
Use additional sheet if necessary)
12.1 Welding, Grinding and Cutting
Procedures to follow for welding, grinding & cutting:
12.1a Remove combustible & flammable materials,
12.1b.Prepare & inspect welding machines, cables, plugs, outlets, ppe etc,
12.1c.Only competent workers shall be allowed to engage hot works.
12.1d Provide signages like hot works ongoing, no smoking etc,
12.1e.Conduct tool box meeting prior to commence hot works activities,
12.1f. Provide fire extinguisher,
12.1g. Isolate the working area by providing barricade to limit access,
12.1h. Keep hot work area clean and free from stagnant water and
12.1i. Wear proper & correct PPE for hot works such as welding helmet, welding apron, face
shield, face mask, welding gloves and others
12.1j. Do housekeeping after works
12.2 Hand and Power Tools
Procedures to follow for hand & power tools:
12.2a. Only competent persons should use hand & power tools,
12.2b. Prepare & inspect tools before & after use,
12.2c. Do not use improvised hand & power tools,
12.2d. Use hand & power tools according to its purpose only,
12.2e. Initiate tool box meeting prior to commence work,
12.2f. Do not use defective & damage hand & power tools,
12.2g. Keep the hand & power tools clean during & after use,

12.2h. Keep power & hand tools properly
12.2i. Use appropriate ppe according to the hand & power tools design & purpose.
12.2j Improvised hand tools or power tools should not be used on site.
12.3 Safe Handling of Materials
12.3a. Arrange or store materials according to its category or class,
12.3b. Provide proper access & egress,
12.3c. Stock pile should be stored of at least 1 meter high,
12.3d. Consider temperature requirement for each material during storage,
12.3e. Maintain the area clean & tidy without water stagnant as possible,
12.3f. Isolate & provide signages & barricade,
12.3g. Provide piece of wood at the bottom of materials for lifting purposes,
12.3h. Use mechanical or machines in lifting heavy materials that requires lifting aid &
12.3i. Do manual lifting for light materials
12.4 Electrical Works and Installation
12.4a A zero current policy shall apply during electrical works & installation,
12.4b Only certified electrician shall engage electrical works & installation,
12.4c Tools to be inspected prior to its use,
12.4d Tool box meeting shall be conducted prior to start the activity,
12.4e Correct PPE to be worn during electrical works & installation,
12.4f Working platform & ladder shall be used for working at height electrical works &
12.4g Inspection to be conducted by competent electrician prior to energization,
12.5. Chipping
12.5a Only an experienced worker shall be allowed to use & operate chipping power tools.
12.5b Inspect power tools, cords & cables before & after use.
12.5c Select appropriate chipping tool for the task to be done.
12.5d Use appropriate & free from damage concrete drill bit or cutting disk.
12.5e use appropriate PPE such as earplug, rubberize hand gloves, trouser, safety shoes, face
shield & mask.
12.5f If chipping to be done at height, ground must be protected with barricade, signages &
spotter to avoid falling debris.
12.5g Use full body harness as needed or when chipping above 6ft is to be executed.
12.5h Do housekeeping after works.
13.0 Enumerate/describe other safety procedures in accomplishing the project.
13.1. Securing Perimeter- perimeter shall be secured by provision of fence to avoid entry of
passersby or non-construction workers.
13.2. Construction Signages- construction signages shall be provided at the entrance of
the two storey building to give warn to the public from staying or entering the perimeter. Signages as
follows but not limited to:

1. Caution! Ongoing Construction.
2. Private Property, No Entry
3. Others
13.3. Excavation- Manual and mechanical excavation shall be done with the following
procedures:
13.31. Manual excavation- shall be done in accordance with the proper use of digging tools
such as shovel/spade, pick axe, digger bar, wheel barrow, sacks or plastic paint container for hauling
of excavated soil. Etc. Procedures as follows:
1. Selection of experience and fit workers for manual excavation,
2. Tool box meeting for the workers involve in the excavation
3. Preparation, selection & inspection of digging tools
4. Clearing & provision of barricade.
5. Manual excavation using digger bar, shovel, spade & pick axe as applicable,
6. Removal & hauling of excavated soil into the paint container or sack,
7. Removal of water (if any) using paint container.
8. Provision of access stair or ladder as necessary
9. Covering of excavation using plywood or the same materials.
13.3.2. Mechanical excavation- shall be done in accordance with the proper use of mini
excavator machine and rotary hammer drill with the aid of digger hand tools such as shovel/spade,
pick axe, digger bar, wheel barrow, sacks or plastic paint container for hauling of excavated soil etc.
Procedures as follows:
1. Selection of experience operator for mini excavator & hammer drill,
2. Conduct of tool box meeting for the workers involve in the excavation,
3. Preparation, selection & inspection mini excavator, hammer drill and digging tools
4. Appointment of spotter or signal man for mini excavator activity,
5. Clearing & provision of barricade.
6. Excavation proper using mini excavator or hammer drill as needed,
7. Manual excavation using digger bar, shovel , spade & pick axe as applicable,
8. Removal & hauling of excavated soil using mini excavator,
9. Removal & hauling of excavated soil into the paint container or sack,
10. Removal of water (if any) using mini excavator or paint container.
11. Provision of access stair or ladder as necessary
12. Covering of excavation using plywood or the same materials.
13.4. Working at heights- shall be implemented in accordance with Occupational Safety &
Health Standard (OSHS) set by the Philippine government.
Working at height procedures:
1. Erection of working scaffold to be done by competent persons,
2. Access like stair or ladder should be provided,
3. Railings and planks to be completed prior to use work scaffold,
4. Sole and base plate to be provided,
5. Work at height at about 6 meter high should be provided with complete work scaffold, full
body harness and life line,
6. Work at height below 6 meters high should be provided by with complete scaffold,

7. No workers should stay under working scaffold with current activities,
8. Only competent workers shall be allowed to do work at height,
9. Tool box meeting shall be done prior to commence work,
10. No throwing of materials or any objects that may fall and cause personal injury,
11. CHB or other materials basket shall be used in bringing up materials,
12. Regular inspection by competent person shall be executed,
13. Dismantling of work scaffold shall be done from top to bottom.
14.0 Penalties/Sanctions – as a matter of policy and to ensure that the safety and health rules
specified herein are complied with, the owner recommended the following sanctions/penalties for any
violations of CSH program(Please attach company policy on penalties, if there are any). :
First offense - Verbal warning
Second offense - 2nd warning
Third offense - 3rd warning
Fourth offense - 1 day suspension
Fifth offense - 3 days suspension
Sixth offense - Dismissal
15.0 Safety, health & security violations
1. Violation to any safety & health protocols of IATF
2. Working with covid-19 like symptoms
3. Deliberate working while infected with Covid-19
4. Littering, loitering & vandalism
5. Verbal abuse
6. Physical fighting
7. Smoking on prohibited areas
8. Horse playing
9. Bullying
10. Stealing
11. Insubordination
12. Unauthorized entry
13. Refusal to do housekeeping or cleaning
14. Unauthorized removal of an installed materials, signages etc
