Module Overview:

This module is designed for Criminology students familiarize the


fundamentals of report writing. It aims to help learners familiarize with
how reports are written, as it points out to the fact that report plays an
important role in the success of any investigation. Included in this unit are
the points to consider in writing reports, the strategies in preparing good
reports, and the types of report writers. One of the basic works of an
investigator is writing reports, and with that the importance and purposes
of police report writing is also elaborated.

Module Outcomes:
At the end of the module the students should have;

 interpreted different reports prepared by an investigator;


 differentiated types of investigative report;
 followed the points and procedures, and additional guidelines in
writing different reports.

LESSON PROPER

Different Types of Report prepared by an investigator

 Spot Report – this is a verbal or written report done with twenty-four hours after an important
incident. This report is written to inform an immediate chief or those in higher position of
particular occurences in his command of responsibility. This report should be acted upon within
24 hours. In some cases, a spot report maybe in form of a radiographic message if the reoortung
unit is far from the receiving officw. The following are indicated in a radiographic message:
Originating office, Addressee, Cite numbers, Precedence actions, Precedence info, Date-time
group, Text written in capital letters.
 Progress Report – this is a written narration of facts developed by the investigator in the course
of the follow-up investigation. This is a report on subsequent details, which are vwery vital to the
case but have not been incorporated in the initial report. This report should be submitted within
three days after the initial report has been submitted, and consequently a monthly report
thereafter.
 Final Report – a complete written narration of facts based from a thorough investigation of the
case. This is a result of evaluation, summation, and analysis of all facts and circumstances of the
case, and the complte accumulation and collection of data.

2. Please be informed that on or about 4:30 o'clock in the afternoon on September 27, 2020
inside Room $503, University of the Cordilleras, Gov. Pack Road, Bakun, Benguet, Juan Tamad
y Talaga alias Junjun, 45 years old, single, a 2 year student of the College of Criminology and a
resident at # 34 Honey Rd, Baguio City stabbed three (3) times at his back Emilio Luna y
Aguinaldo, 65 years old, a college instructor of the same school mentioned above and a resident
at #5 Rimando Rd, Baguio City. Initial investigation disclosed that Jose Rizal y Mercado, 35
years old, a third year student of the same college and school and a resident at Poblacion Dos,
Bakun, Benguet was sitting in front of Mr Luna, who was then busy checking quiz papers, when
he saw Juan Tamad, with a Buguias-made double-bladed knife in his right hand, approached Mr
Luna from behind and without any word, stabbed him on his back three (3) times and when the
victim, Mr Luna, dropped on the floor, he (Juan) kicked him on his body two (2) times, spat on
his face and ran away while saying, "dayta ti gungunam nga babaero." Investigation further
disclosed that the suspect has been suspecting that the victim and Culassa, his wife, have had an
illicit sexual . Investigation finally disclosed that the victim was brought to the Baguio General
Hospital by concerned students but he was declared dead on arrival by the attending physician

Disposition: case is under follow-up investigation Officer on case PEMS Don Pedro

2. Please be informed that PEMS Primitivo Simplicio, when he led a team of criminal
investigators to the crime scene regarding the stabbing incident happened on or about 4:30
o'clock in the afternoon on September 27, 2020 inside Room $503, University of the Cordilleras,
Gov. Pack Road, Bakun, Benguet, was able to collect sets of fingerprints as well as blood
samples. In addition, the team also recovered inside a trashcan on the hallway at the second
floor of S building the alleged weapon the suspect used in the crime - a 20-inch Ibaloi-made
double-bladed knife. These pieces of evidence were submitted to the PNP Regional Crime
Laboratory Office, Camp Dangwa, La Trinidad, Benguet for examination

2 After this office has gathered all necessary pieces of evidence and the testimonies of the
witnesses, a case of Murder defined and penalized under Article 28 of the Revised Penal Code

suspect Juan Tamad y Talaga alias Junjun, 45 years old, single, a 2 year student of the College of
Criminology and a resident at #34 Honey Rd, Baguio regarding the stabbing incident happened

ACTIVITY

Activity 1: Write the Spot Report base on the contents of the news story below, for information
that are not found in the news you may supply your own Fictional data to complete the report.
Write your report in yellow pad paper (30 points)

At about 1:00 A.M. of February 22, 2019 (Friday) a shooting incident transpired at Brgy.
Omanod, Sta. Catalina, Negros Oriental victimizing Victor Rapada y Enojo, 52 years old, male,
separated and resident of Brgy. Omanod, Sta. Catalina, Negros Oriental. Initial investigation
was conducted and Eno Rapada y Enojo (cousin of the victim) revealed that the victim was at
the waiting shed beside the national highway when minutes later they heard several gun burst,
when they verify they saw the victim lying on the ground wounded. Victim sustained two (2)
gunshot wounds on the left side of his chest that cause his instantaneous death from
unidentified suspects. Recovered at the crime scene were One (1) empty shell of cal. 9mm, one
(1) empty shell of cal. 45 and one (1) slug of cal. 9mm. a follow investigation still conducted by
Sta. Catalina Police Station for possible identification of the suspect.
