past few days. Your immediate supervisor has given you multiple verbal warnings, yet you
continue to take extra long breaks. Our Company does not tolerate this behavior as it will

This letter serves as a written warning of your misconduct and please be advised that
if you continue to do so – as per company policy ______.

As this is your first warning in this matter, we are not taking any disciplinary action.
