Gastric Juice Analysis

The document discusses gastric juice analysis and gastric secretion stimulation. It describes the major constituents of gastric juice as water, hydrochloric acid, and electrolytes. It also lists several enzymes produced in the stomach, including pepsin, lipase, rennin, gastricsin, and mucin. It then discusses 10 different test meals that are used to stimulate gastric secretion, including Ewald's test meal, Riegel's test meal, and Lavine's alcohol test meal.

GASTRIC JUICE ANALYSIS - soft rubber, plain catheter tip, w/ several

- Major constituents openings

 Water - swallowed or passed through the nostril
 HCl - marked @: 22-24 ‘’ L w/ a tape
 Inorganic constituents (electrolytes) - inserted after application of 1% cocaine HCl and
- Major Organic constituents the tube lubricated w/ mineral oil or a few drops
Enzymes such as: of glycerine
 Pepsin - syringe or Wangesteen suction apparatus may be
 Lipase used in collecting
 Rennin
 Gastricsin Note:
 Mucin
Other kinds of tubes:
Sources of Gastric Juice  Lyn tube
1. Chief/ Peptic cells
 Einborn tube
- Producing the protein-splitting enzyme pepsin  Jutte tube- has a stylet
2. Parietal/ Oxyntic cells  Miller-Abbot tube- has mercury @ the tip
- Producing the HCl and intrinsic factor and w/ SG of 13.6
(erythropoietic factor) of Castle (absence leads to
anemia) STIMULANTS FOR GASTRIC SECRETION
3. Goblet mucous secreting cells A. Test Meals:
1. Ewald’s Test Meal (bfast)
o Gastric glands  40 g bread & 400 mL water/ weak tea
-produce lipase (fat splitting enzyme w/c is w/o cream or sugar
active in alk. medium and inactive in acid  commonly used for routine gastric juice
medium) and rennin (enzyme that causes exam’n
curdling of milk) 2. Riegel’s Test Meal
 100-150 g mashed potatoes, 150-200 g
Types of Evacuation Tubes broiled beef steak & 400 cc of bouillon
1. Ewald’s or Boa’s Tube soup
- moderately hard but flexible rubber tube, about  mainly used to detect hypoacidity or
12 mm, inserted via propulsion achylia
- useful in case of poisoning 3. Boa’s Test Meal
- best used for short duration of evacuation and  1 tbs rolled oats in 1 qt of water, broiled
inexperienced patients to 1 pint & added w/ a pinch of salts
2. Rehfuss Tube  ideal to detect the presence of lactic
- soft rubber, about 3-4 mm (diameter) and acid in the gastric juice
swallowed by the aid of gravity  withdraw 15mins- 1hr after given
- 36’’ L and has three markings: 17, 23, & 30 ‘’ L 4. Heckman’s Test Meal
- catheter-like w/ gold or silver metal at the tip  80 ml freshly prep. egg albumin, 130
- collection via syringe attachment and duodenal mL dist. water & 2 drops of 2% sol’n of
contents collection methylene blue
3. Sawyer Tube  Dist. water + 4 g Witte’s peptone ->
- mod. soft rubber, 3-4 mm in diameter and 86 cm heat @ body temp -> filter
in L  easier to swallow
- three markings: 49, 65, & 81 cm in L 5. Dock’s Test Meal
- introduced via propulsion, usually connected to a  shredded whole wheat biscuit instead of
syringe for aspiration bread (lactic acid & yeast cells)
4. Kaslow Tube  modification of Ewald’s
- mod. soft rubber about 3-4 mm in diameter and 6. Fischer’s Test Meal
56 ‘’ L  Similar to Reigel, consisting of Ewald
- three markings: 18, 26, and 30 ‘’ L meal + 1 lb lean, chopped hamburg
stock, broiled and lightly seasoned
- same intro w/ Sawyer
 higher acidity values
 Fischer and Reigel meals could be
removed in 3-4 hrs after
5. Levin Tube

7. Lavine’s Test Meal (Alcohol)
 50 cc of 7% ethyl alcohol
 2% methylene blue/ methylthionine
chloride sol’n- added for more accurate
determination
 detects regurgitation of alkaline
material from the duodenum
8. Motor Test Meal
 spinach or raisins + 400 cc of water
9. Salzer Motility Test Meal
 1 ounce of roast beef or broiled
lambchop. 250 mL of milk. 50 g boiled
rice, and 1 soft boiled egg
 give an Ewald’s meal 4 hrs. later
 meat fibers & etc. from the first meal
indicate deficient gastric activity
10. Statis Meal
 2 ounces of half cooked rice and 12
well-chewed raisins
 Aspirate the stomach in AM and wash
w/ little dist. water-> examine for rice
starch and particles of raisin
 Supplanted by barium meal and
fluoroscopic exam’n
 simpler than Salzer meal
