Completing the report
The file to be completed is named “Report Template” and is a word document.
1 Cover Page & Preliminary Information
a. Replace Vessel name in cover page with actual Vessel Name
b. Replace “Vessel Name” in Page 2 header with actual vessel name
c. Complete Inspection details in Page 2
d. In Executive summary, include general summary of attendance with main deficiencies, best
practices observed, crew behaviour, any additional notable event throughout the
attendance.
2 Checklist
a. Complete the checklist questions by selecting with an X the appropriate column.
b. If a question leads to a raised risk, observation or deficiency (or when the question is N/I or
N/A), please expand and justify in Remarks section.
3 Deficiencies
a. Add all deficiencies arising from the inspection.
b. Please also include any deficiency which was raised from the checklist, and any other
noticed throughout the attendance.
c. We expect a very strict inspections writing down all observations, minor and major, and a
well elaborated deficienciy list.
d. Be as specific as possible when describing each finding including: location, severity, detailed
description
