JOINT AFFIDAVIT OF TWO DISINTERESTED WITNESSES
WE, AURORA V. APOSTOL and ANITA S. ELINZANO, 72 and 75 years old respectively, Filipino citizens, widow and
married respectively, and both with residence and postal address at Teachers Village, Brgy. Pogonlomboy, Mangatarem,
Pangasinan, having been duly sworn to an oath in accordance with law, do hereby jointly and severally depose and say:
THAT we personally know CHARLIE RETOTAR ANCHETA of Teachers Village, Brgy. spouses Pogonlomboy,
Mangatarem, Pangasinan to have been born in Mangatarem, Pangasinan on DECEMBER 22, 1966 to Virgilio Tanaoit
Ancheta and Carmelita Danden Retotar;
THAT we likewise know that he is long known in our community as CHARLIE or CHARLITO who died on OCTOBER
11, 2022 who was married to MYRA B. RUANTO ;
He got sick when he was a child, that the name CHARLIE was given to him as his nickname;
THAT being residents and mutual friends in the same community, we have had many occasions and opportunities to
learn and take note of his date of birth which is DECEMBER 22, 1966 and his parents being Virgilio Tanaoit Ancheta and
Carmelita Danden Retotar;
THAT the CHARLITO RITAR ANCHETA with SSS MEMBERSHIP ID NO. 02-1581576-3 (hereto attached); and
CHARLITO RETOTAR ANCHETA who died on DEC. 11, 2022 registered as deceased with the Municipal Civil Registrar of
Mangatarem, Pangasinan under REGISTRY NO. 2022-387 (hereto attached); and CHARLIE RETOTAR ANCHETA who was
born on DEC. 22, 1966 as per hereto attached Birth Certificate issued by the Municipal Civil Registrarof Mangatarem,
Pangasinan with LCR CIVIL REGISTRY NO. 1091, CHARALITO R. ANCHETA in Marriage Certificate issued by the Local civil
Registrar of Mangatarem, Pangasinan, refers to one and the same person who is the subject matter of this document;
That the name CARMEN was her nickname and she used it since childhood;
In the Birth Certificate of CHARLITO, his mother’s name recorded as “CARMEN”;
THAT the CARMELITA DANDEN RETOTAR in DEATH CERTIFICATE NO. 2022-387 and CARMELITA RETOTAR
appearing in CERTIFICATE OF MARRIAGE with Registry No. 952 (copy attached) also refers to one and the same person and
the lawful mother of deceased member CHARLIE/CHARLITO RETOTAR ANCHETA;
THAT there is/are no other person/s in our community that we know of who is/are identified and responds to the
name CHARLIE/CHARLITO RETOTAR ANCHETA nor to the name CARMELITA/CARMEN DANDEN RETOTAR except those
persons or individuals referred to in the above narrative;
THAT we are executing this affidavit to prove and attest to the foregoing facts and circumstances related to
CHARLIE/CHARLITO RETOTAR ANCHETA and to CARMELITA/CARMEN DANDEN RETOTAR;
THAT the foregoing are true and correct and executed to the best of our honest knowledge, information and belief;
