BACKGROUND OF THE ACTIVITY
Activity Title SLAC on Individual Learning Monitoring Plan
Date December 17, 2020

Participants Learning Facilitators, Principal 2 of Erenas Elem. School All Teachers of
Erenas Elementary School
Description In its commitment to ensure teaching and learning continuity while
looking after the health, safety, and well-being of its learners, teachers,
and personnel, the Department of Education (DepEd) issued DepEd
Order No. 012, s. 2020 titled Adoption of the Basic Education Learning
Continuity Plan (BE-LCP0 for School Year (SY) 2020-2021 in light of the
COVID-19 Pandemic.
To supplement the said DO, the DepEd issues the Interim Policy
Guidelines for Assessment and Grading in Light of the Basic Education
Learning Continuity Plan (Enclosure No. 1) to provide guidance on the
assessment of student learning and on the grading scheme to be
adopted this school year.
As DepEd pursues learning continuity, it is imperative for schools to
take stock of assessment and grading practices that will most
meaningfully support learner development and respond to Varied
contexts at this time.
Objectives The following were the specific objectives of the conduct of the activity.
a. discuss the Interim Guidelines in accomplishing Individual
Learning Monitoring Plan.
b. gain common understanding on accomplishing Individual
Learning Monitoring Plan
c. address other issues, gaps and concerns in accomplishing
Individual Learning Monitoring Plan.
HIGHLIGHT OF THE ACTIVITY
Impact/Results The identified objectives and target for the activity were met.
1. Were able to discuss the Interim Guidelines for Assessment and
Grading in Light of the Basic Education Learning Continuity Plan
2. Where able to gain common understanding on the
contextualization of the assessment tools to be employed in
determining the learning outcome of the learners; and
3. Where able to address other issues, gaps and concerns related
to curriculum implementation.
POST ACTIVITY EVALUATION
Participants’ Feedback The teachers were very happy on this SLAC Session since they were
properly guided and equipped with information about the SLAC Session
on DepEd Order # 31 (SCHOOL ON-SITE CONFERENCE ON THE INTERIM
GUIDELINES FOR ASSESSMENT AND GRADING IN THE LIGHT OF THE
BASIC EDUCATION LEARNING CONTINUITY PLAN). This will help the
teacher address the challenges in these new modalities of learning.
Methodology:
The physical conference or face to face Orientation was being conducted
between Erenas and Bulao Elementary School Teachers, observing
proper protocol on social distancing and the like. The Orientation of
DepEd order # 31 (SCHOOL ON-SITE CONFERENCE ON THE INTERIM
GUIDELINES FOR ASSESSMENT AND GRADING IN THE LIGHT OF THE
BASIC EDUCATION LEARNING CONTINUITY PLAN) were conducted and
mentored by the Learning facilitators presenting all the needed
information.
Management of the Activity:
The said activity was managed by the following Learning Facilitators:

All the materials, equipment food expenses were taken from the MOOE
fund.
RECOMMENDATIONS/ It is recommended that all teachers of Erenas and Bulao
ANALYSIS Elementary School would apply all the learning and
knowledge they acquired on this On-Site Conference
