SWORN STATEMENT
I, Ralph Frios D. Dipol, of legal age, Filipino, a resident of Purok 4, Barangay Rajah Muda, Tacurong
City, Sultan Kudarat after being duly sworn in accordance with law, hereby depose and state that:
1. I am the manager of Mang Inasal, Tacurong Branch.
2. On the 15th of January 2009 at around tree thirty in the afternoon, I was on duty when the
theft happened.
3. The victim of the incident identified as Janice Acero, our regular costumer, residing at
barangay Buenaflor, Tacurong City, Sultan Kudarat.
4. The suspect of the incident identified as Anthony Guerrero, ex-husband, resident of Barangay
Katungal, Tacurong City, Sultan Kudarat.
5. At first Anthony is following Janice up until Janice came inside of Mang Inasal and place an
order, after that Janice sit on her favorite spot.
6. When Janice takes a restroom, Anthony takes a chance also to steal Janice things, especially
the cash. The suspect quickly escapes the scene after the incident.
7. I am executing this affidavit to attest to the truth and veracity of the foregoing declaration and
for whatever purpose this may lawfully serve.

ASSIST HER IN THE GIVING OF STATEMENTS
Mr./ RALPH FRIOS DIPOL hereby volunteer yourself to give sworn statement about your personal knowledge
about theft incident that transpired on January 15,2009 at 3: 30 pm.
Before you will give your sworn statements, I would like to inform you of your rights enshrined under the 1987
Philippine Constitution, as follows:
1. That you have the right to remain silent and any information that you will give can be used as evidence
against you in any court of the Philippines;
2. That you have a right to have counsel preferable of your own choice, if you cannot afford the services of the
counsel, the Government of the Philippines will give you one to assist you;
3. That you have the right to know why you are here in our office; and
4. That you have the right to be informed of all your Constitutional rights enumerated above.
QUESTIONS: After you were informed of all your Constitutional rights, are you welling and voluntarily sign your

ANSWER It happened at Mang Inasal, Tacurong Branch at 3:30 in the afternoon
06. QUESTION Who were involve in this incident?
ANSWER Mr. Anthony
07. QUESTION How theft happened?
ANSWER When Janice takes a restroom, Anthony takes the chance to steal the things and cash that
belongs to Janice.
08. QUESTION Do you know the victim?
ANSWER Yes sir, she is our regular costumer.
09. QUESTION To clear this investigation can you tell what really happened in the incident?
ANSWER I saw Anthony following ma’am Janice from outside till ma’am Janice sit on her favorite
spot.
10. QUESTION What kind of strategy he used to escaped?
ANSWER He quickly settled his hoodie jacket and walk fast.
11. QUESTION Aside of you is their anyone seen that incident?
ANSWER Yes. My one staff saw that incident.
12. QUESTION What did you do after you witnessed that incident?
ANSWER I called the security team. But then the suspect escape so fast.
13. QUESTION Are you sure of what you’ve seen?
ANSWER I really sure of that.
14. QUESTION For the mean time I don’t have further questions, do you have any statements to add or
take away on this affidavit?
ANSWER Nothing sir for the meantime.
15. QUESTION Were you threatened, promised or bribed to do this confession?
ANSWER No sir. I voluntarily made this for the peace and to help our regular costumer to the
justice that she really deserves.
