SUBJECT: SPOT REPORT RE-STABBING INCIDENT (sample)

1. On January 20, 2012 at about 1:30 P.M., a certain Dina Makabangon, 24 yrs
old, resident of Campsite, Brgy Guinabot, Midsalip, Zamboanga del Sur, appeared
before this office to report a stabbing incident involving her husband, Froyo
Makabangon, as the victim.
2. According to the complainant, at about 9:00 A.M. of same date, she saw her
husband having a drinking spree outside of their house with of his friends who are also
their neighbors, identified as Aga Otot and Toto Mola. Not long after, she heard
someone shout invectives, followed by a commotion. Immediately, she rushed to the
scene where she saw Abella and bartoloy mauling her husband. She pleaded for the
suspects to stop but her plea fell on deaf ears.
3. Instead, she saw Bartoloy draw a bladed weapon from his waistband and
repeatedly stab his husband in the chest and different parts of the body. After which, the
suspects fled on foot.
4. With the help of Samaritan neighbors, they rushed her husband to the nearest
hospital where he was declared dead on arrival by the attending physician.
5. The undersigned immediately dispatched a team led by SPO4 aballe to
investigate the case, determine the suspects’ whereabouts and bring them to the bar of
justice.
