Greetings!
We, the undersigned, third year Bachelor of Science in Information Technology students at
Cavite State University, Indang, Cavite, are tasked to conduct research for our subject
Methods of Research. The purpose of our research is to develop a multiplayer game that acts
as a supplementary learning tool to help students understand living things.
In view of this, may request your permission to allow us to conduct our research with your
organization as our target beneficiary. The study involves the conduct of an interview with
your 10 Science teachers and a survey with 50 grade 4 students and will be conducted
following your school’s guidelines and ethical standards and Data Privacy Act of 2012 or
Republic Act 10173 at their convenience. In addition, parental consent will be secured from
parent (or guardian) prior to survey with the students. A copy of the interview and survey
questionnaires, and sample parental consent are herewith attached for your reference.
Rest assured that the research processes will not disrupt normal operations and will fully
respect the confidentiality and privacy of all participants. Further, all data collected, including
the personal information of the identified respondents, will not be published publicly and will
only be used for research purposes.
