In case to case basis (for transferee) For Grade 10 and 12 Level, Student Evaluation Form shail be presented to further evaluate students’ status. 5. _ SHS School Heads are required to submit to the Division Planning Officer (Attn: Mrs. Agustina R. Albiso) a certification of their offerings for validation against LIS. Submission of certifications from the SHS School head shall be made between May 6-13, 2024. The validated certification will be a part of the documents to be presented during the checking. 6. Guidelines on the preparation and checking of School Forms are based from the DepEd Order 11, s. 2018. Any document that is not compliant to the format will not be recognized nor accepted. 7. Guidelines in Checking of Diploma and Certificates are the following: > Diplomas and certificates of public schools shall bear the initial of the Education Program Supervisors prior to the signing or acquiring the signature (through facsimile) of the Schools Division Superintendent (private schools may use the template of the Diploma /Certificate of DepEd including signatories, or may opt to have their own as long as citations and data are in adherence to DepEd Order No. 11. The composition of the committee on the checking and validation of school forms both for public and private schools are as follows in which both the division and district will decide as to when will be the specific date or schedule of the district (between February 17-28, 2025 for Private Schools and March 1-31, 2025 for Public Schools) 9.1. Division - Division Field Technical Assistance Team (DFTAT) 9.2. District - Public Schools District Supervisor (PSDS), District Guidance Coordinator and school head from the secondary level preferably that can represent the strand offering of the school or the senior high schoo! focal person. 10. Travel and other incidental expenses of school personnel involved in the checking and validation is chargeable to Local School MOOE Fund, while the Division Personnel is chargeable to the Division Fund, subject to its availability and the usual accounting and auditing rules and regulations. 11.
