**Consent Form for Participation in the Study on Plagiarism Awareness Among Grade 11

STEM Students**

You are invited to join a research study by the STEM 11-C class to assess plagiarism
awareness among Grade 11 STEM students.

The objectives of the study are to:

1. Measure the level of Grade 11 STEM students' awareness of plagiarism.


2. Identify students' attitudes and behaviors related to plagiarism, including whether they
engage in it and their reasons for doing so.
3. Examine students' ethical and academic perspectives concerning plagiarism and its
consequences.
4. Assess the effectiveness of existing plagiarism awareness efforts within the educational
institution.
5. Determine the factors influencing plagiarism among students, such as the difficulty of
assignments, time constraints, and lack of confidence.
