Describes the specific activities that will take place to achieve the
objectives
Answers the questions HOW, WHEN and WHY?
Visualize the implementation scheme of the project
Impact of proposed activities
Expected output and outcome [how community will benefit from
project]
Long-term strategies for maintaining the on-going project
c. Staffing and Administration
Discusses staffing needs, number of staff needed, qualification and
specific assignments
d. Evaluation Procedures [1-2 pages]
Describes the manner in which evaluation information will be
collected and how the data will be analyzed.
Presents your plan for how the evaluation and its results will be
reported and the audience to which it will be directed
Two types of formal evaluation: one-measures the product or
service and two analyzes the process.
Provide methods-criteria, data, instruments and analysis
e. Sustainability
Demonstrate in very concrete ways the long-term financial viability
of the project to be funded.
Contains proofs for capability building and future self-sufficiency to
expand services and operation.
Describes specific current and projected events and trends that will
affect funding source
f. Budget Proposal [1-2 pages]
Statement of projected expenses and costs of the project
Cost should be grouped into sub-categories reflecting critical areas
of expense [i.e salaries, travel, equipment, facilities, rent and
printing]
III. Organizational Information [1 page]
Attach a brochure or prepared statement of the agency’s [or
community organization] vision, mission and goals statements.
In one page describe, structure, programs and expertise of the
organization.
Give information about board composition and their functions
including volunteers.
IV.CONCLUSION [1/2 page]
In one paragraph or two, call attention to the future after the grant
or funding is completed
Outline some of the follow-up activities that might be undertaken to
prepare your funders for your next request.
Make a final appeal for your project
Underscore why your agency needs funding to accomplish it.
Use a bit of emotion to solidify your case.
