SUBJECT : Final Report Re: Shooting Incident Resulted in the Death of Julius
Madriaga

1. This has reference to Memorandum Spot Report Re: Shooting Incident Resulted to the Death of Julius
Madriaga dated 18 November 2021 and Progress Report dated 23 November 2021.

2. On November 18, 2021, at about 3:00 P.M. a shooting incident transpired at Gapan City Plaza, wherein
victim Julius Madriaga 25 years old, married, tricycle driver, a resident Brgy. San Lorenzo, Gapan City
was shot by suspect Gerald Palomo, 28 years old, married, a utility worker, and a resident of the same
place.

3. In the afternoon of 18 November 2021, an informant named Daira Nepomoceno called the Gapan City
Police Station to report the incident. Immediately, PO3 Earl De Jesus, together with PO1 Rhica Clemente
and PO1 Justine Liwag responded to the place of incident where they found a commotion of people.

4. The victim sustained multiple gunshot wounds in the different parts of the body and was rushed to
Good Samaritan Hospital for medical treatment but was pronounced dead on arrival (DOA) by the
attending physician.

5. The suspect Gerald Palomo is currently detained at Gapan City Police Station and a case of Homicide
in violation of Article 249 of the Revised Penal Code is pending trial.

6. The case is already raffled at Branch 45 of Gapan City Municipal Trial Court.

7. The accused Gerald Palomo was arraigned on 30 November 2021 where he pleaded Guilty and the
case was sent for pre-trial on 3 December 2021.
