Group-Medi claim Policy Guidelines
GMP Policy is bifurcated in 2 Policies (Employee & Parents Policy).
1. Employee Policy: This policy covers (1+3) self, spouse, and dependent children.
The sum insured for the family under this policy is Rs. 3 lakhs, provided on a floater
basis.
2. Parents Policy: This policy covers only parents. The sum insured for the family
under this policy is also Rs. 3 lakhs, provided on a floater basis.
Room Eligibility – Room – 2 % of Sum Insured. ICU- 3% of sum Insured.
New Joining- Every new employee joins in the organization should submit his
/ her family particulars in the prescribed Medi-claim enrollment form with
passport size photos pasted on it along with date of birth proofs. Newly joined
employee and his family members will be enrolled in to Group Mediclaim Policy
after successful running of his / her first pay in the consecutive month.
In-term New Dependent Endorsements
If any new dependent addition in the family like NEW BORN BABY or
NEWLY MARRIED SPOUCE should be endorsed within 30 Days from the day
of birth or from the day of married.
Documents to be submitted for New Dependent Enrollment
New Born Baby – 1. Medicalim Enrollment form duly filled.
2. Date of Birth Certificate issued by
Municipal Authorities / Hospital
/Aadhar.
Newly Married – Spouse 1. Medicalim Enrollment form duly filled.
2. Marriage Certificate / Updated Aadhar
card with Spouse name.
3. Date of birth Proof of Spouse.
The details are to be submitted to Medi-Claim Help desk – Head office-
Hyderabad, immediately in original. And also send a scan copy of the same to
[email protected] mail id for processing.
Note:
1. If the Employees fail to provide the dependent details at the time of joining
they cannot be enrolled in the current year Medi-claim policy. Further they will
be endorsed in the coming year Medi-claim policy only. For the same they have
to submit all the details as said above.
2. Once Employee resigns/leave the company, Group Mediclaim Policy benefits
will not be applicable from the day of leaving the company. It will be discontinued
when you leave the company.
Cash-Less Facility
For cashless hospitalization along with your Group Medi-claim card please do
carry a Photo ID proof which is a mandatory document.
One of the basic requirements of insurance is Intimation. It simply means
intimating the Medi-Assist TPA about the hospitalization immediately within 24
hours of the hospitalization by calling to the toll free number present on the
Medi-claim card.
Reimbursement
If you are applying for Reimbursement, you have to submit the duly fill in Claim
form along with All Original Documents, Investigation reports, complete bills and
cash paid receipts as specified in the check list which is attached. The duly fill
in claim document should be submitted within 15 working days from the
day of discharge from the hospital either directly to the Medi assist India
Pvt Ltd (or) sending the documents to Mediclaim Help desk situated at our
Head Office Hyderabad.
Reimbursement claims can be submitted to the Medi assist TPA through
registered post or courier or can be handed over directly at any of the branches.
Non-Coverages
1. Any type of Cosmetic, Maternity & Robotic are not covered under our
Group Medi-claim Policy.
2. Less than 24 hour’s hospitalization is not covered. Hence minimum 24
hours of hospitalization with active line of treatment is mandatory.
3. By Birth External Congenital diseases are not covered.
4. Treatment for infertility is not covered.

Online E-Card Download Process:-
Now all the employees have the facility to download their medical E-card directly.

To login you have to give user ID and password in the following format

After logging into the page you have to fill your own details as shown in the
below image.
Next you have to change your password according to the given guidelines as shown
in the below image.
Please do remember that after changing your password only you can able to
download or view your own medical E-card.
After submitting rest of the details like BANK DETAILS & IDENTIFY PROOF
Please re-login in to your page with new password given by you then you can
download or View your own medical E-card.
Please do remember or save the password given by you; once it is lost it
cannot be resent again.
