INSTRUCTIONS AND GUIDELINES FOR FILLING OUT THE FORM
1. General Information:
o Fill in the name of the Barangay, Municipality, and Province where the data is being collected.
o Indicate the reporting period (e.g., monthly, quarterly, or yearly).
2. Demographic Data:
o Total Population: Enter the total number of residents at the beginning and end of the period.
o Male/Female: Indicate the breakdown of the population by gender.
o Households and Families: Count the total number of households and families.
o Houses: Report the number of houses in the area.
o Vacant and Livable Houses: Report the number of vacant and livable houses in the area.
3. Population Movement:
o In-Migrant: Record the number of people who moved into the barangay.
o Out-Migrant: Record those who left the barangay.
o Births and Deaths: Count all registered births and deaths.
o Net Population Change: This is calculated as Births + In-Migrants - Deaths - Out-Migrants.
o Transferee: Count those who moved within the same municipality or province but changed barangays.
4. Marital and Family Status:
o Total Married: Indicate the total number of legally married individuals.
o Marriages: Report the number of new marriages during the period.
o Live-In (18-24 y/o) & (25 y/o & Above): Track the number of cohabitating couples.
o Widow and Single Parent: Record the total number of widowed individuals and single parents.
5. Health and Reproductive Data:
o MWRAs (Married Women of Reproductive Age): Count the number of married women aged 15-49.
o Pregnant Women: Indicate the total number of pregnancies.
o Teenage Pregnancy: Count the number of pregnancies among women aged 19 and below.
o Women Who Gave Birth: Record all childbirths within the reporting period.
o Postpartum Women: Count women within six weeks after childbirth.
o Menopause: Number of women who reached menopause.
o Death (MWRAs), Maternal Mortality, and Infant Mortality: Record all related deaths.
o Tubal Ligation: Count the number of women who underwent permanent contraception.
6. Health and Well-being Data:
o Suicide Incidents: Record the number of suicide cases.
o Mental Health Cases: Track reported cases of mental health issues requiring intervention.
o Substance Abuse Cases: Report the number of individuals with identified substance abuse problems.
o Disability Cases: Count individuals with disabilities in the barangay.
7. Filling Out the Table:
o Beginning Count: Enter the number from the previous reporting period.
o Additions: Record increases in each category (e.g., new births, in-migrants).
o Reductions: Record decreases (e.g., deaths, out-migrants).
o Ending Count: Compute the final number (Beginning Count + Additions - Reductions).
8. Certification:
o The form should be completed by the designated officer.
o Include the preparer’s name, designation, and date.
o Additional notes or explanations can be written in the Remarks/Notes section.
