Procedure For A Statistical Report

This document establishes the guidelines for preparing statistical reports on occupational safety and health. It defines indices such as probability, consequence, and accident rate, and specifies how to calculate them using accident data and hours worked. It assigns responsibilities such as preparing weekly reports and statistics, and stipulates that copies of the monthly reports will be sent to the relevant authorities and management.

1.GENERALITIES

▪ Being of utmost importance for the employer, the evaluation of

compliance with their risk prevention programs and for the
mandatory submission of statistical security indexes to the
state institutions of the state, it is necessary to establish a system
statistical accident recording and evaluation uniform

▪ The method used by the employer for the registration and implementation of the
security indices, has been designed to correspond perfectly
with national and international standards.

2. PURPOSE.

The purpose of this procedure is to establish the basic guidelines.

for the preparation of statistical reports.

objective

In order to measure (evaluate) month by month the development of the process of

risk prevention, with regard to accident statistics
work, it is necessary to keep a record of the most relevant indices in
this subject and they are:

− Probability or Frequency Index (IP).

− Consequence or Severity Index (CSI).

4. DEFINITIONS:

▪ First aid.

It is any injury or illness of the person related to work,

whose treatment is limited to the service of a first aid assistant and that
does not require further treatment by a doctor.

▪ Medical Assistance.

It is any wound or personal injury that requires medical treatment.

another professional.

▪ Accident with lost time (incapacitating).

It is any injury or illness related to work and that

prevents the person from attending work the next business day, but without including

the day the injury or illness occurred. In fatal cases and of

permanent total disability will use 6,000 lost days.

The reportable incapacitating injuries (with medical leave) are four.

classes:

− Death: any death resulting from a work injury

regardless of the time elapsed between the accident and the death.

− Total permanent disability: Any non-fatal injury that incapacitates

to the worker to perform any profitable function.

− Permanent partial disability: Any injury that does not cause the
death but resulting in the functional or anatomical loss of
a member.

− Temporary total incapacity: Any injury that does not cause death, and
without functional or anatomical loss of a limb but resulting in
resulting in one day or more of inability to work.

▪ Probability or Frequency Index:

It indicates the number of accidents with lost time or reportable without

waste of time, occurred and related to a time period of 200,000
hours worked.

Probability or Frequency Index (IP) = Number of accidents x 200,000

▪ Consequence or Severity Index:

It is the number of lost or unworked days by the construction staff due to

effect of accidents relating them to a period of 200,000 hours.
work.

Consequence or Severity Index (CI) = Number of days not worked x 200,000

▪ Accident Rate Index (IA):

This index establishes a relationship between the two previous indices.

providing us with a more logical comparative measure than if we compared
the indices separately.
