Relies on slaclab/pydm#1108

Displays the connection status for both live and archive channels for each curve. This is shown in 3 ways in the curve table, the Live Data or Archive Data cell for the curve will:

  1. Become disabled if the channel is disconnected
  2. Have a red background if the channel is disconnected
  3. Have a tooltip stating that the associated channel is disconnected, if it is disconnected

The checkbox in these cells will still display as checked even if the channels are disconnected, which may cause some confusion, but possibly less than having an empty box because it is hard to tell it's been disabled.
