Narrative Report: Prepared by

The narrative report summarizes the school's efforts to help two students, Hensley Zapanta and Bryan Dave San Luis, continue their education after an undisclosed incident. Alternative learning arrangements were provided to help Hensley pass her classes and promote to 10th grade. Bryan transferred to another school with the support of the child protection committee. Both students were later offered counseling and the option to continue their education.

NARRATIVE REPORT

Set in one clearly defined objective of helping Hensley Zapanta finish Grade 9 in
spite of what had happened to her, recover from the experience and boost her morale
all at the same time, the concerned people purposively continued and responded to
obligation and responsibility of teaching and counselling the latter.
Hensley was provided an alternative workspace for her convenience and
comfort. From modular instruction, Hensley was placed under the tutorial approach. The
said intervention lasted until March. Similarly, Bryan San Luis continued his studies
outside the classroom regular setting.
Hensley passed the tests in all learning areas that gave her higher rating
compared to the last grading period. While Bryan handed in all the activities and
requirements to all his teachers complete and on time.
Similarly, moral and spiritual upholding continued through the time and
commitment rendered by the principal, Mrs. Perlita De Leon and Mrs. Montealegre, PTA
president.
Lately, it was perceived and observed that Hensley has already coped up from
the incidence and did not need a professional counselling.
Hensley got a general average of 82.25 (Approaching Proficiency) and promoted
to Grade 10.While upon interview with Mr. and Mrs. Zapanta just before the school year
ended, it was mentioned that Hensley will be transferring to Pangasinan and Bryan had
decided to transfer in other school. The school handed him the report card and
certificate of good morale last April 2015.
However, it came to our attention last June 5 upon my visit that she did not enroll
and has no plan of enrolling this year. They were invited to the school and had a chance
to talk to the new principal, Mrs. Roquita S. Suyod who convinced them to enroll
Hensley back to school, and that if not emotionally and psychologically ready to return
to regular classroom setting can be accommodated again under Alternative Delivery
Mode- Home based instruction which the parents accepted. On the other hand, Bryan
San Luis transferred to a private school Tomas Claudio Memorial College following the
advice of the Child- Protection Committee last year to provide him a different
environment and at the same time recover from the experience.
( Attached herewith are the letters of confirmation signed by their guardians).
While, upon receiving the endorsement letter, last June 5, an intervention
program were offered to both students involved to give both of them the right to
continue schooling. Since Bryan have already partially paid the tuition fee, they feel it
would be better not to withdraw his enrolment at TCMC.
