I GENERAL REQUIREMENT

1 Mobilization
The Contractor shall arrange and pay for the mobilization of manpower,
temporary plant, warehouse, offices, equipment especially tower crane, site 1 lot
preparation and site layout and the like to complete the Works;
2 Demobilization
The Contractor shall arrange and pay for the removal of the temporary plant,
warehouse, tower crane, offices and the like after the full completion of the 1 lot
Works or as the Owner through the PM deemed that the temprary facilities
are no longer required at the project ;
3 Permits and Licenses
The Contractor shall arrange and secure for the necessary permits required
by LGU including Demolition Permit, Excavation Permit, Building Permit,
Occupancy Permit and ECC for the succesful completion of the Works; 1 lot
Payment will be by Owner

The Contractor shall arrange, secure and pay for the Permit that may be
required by the LGU to operate the System; 1 lot

4 Temporary Offices, Warehouse, Water, Power and Light


Construction Staff are not allowed to stay at the job site after work hours.
The Contractor shall arrange and pay for the lodging facilities of the workers 1 lot
and supervisory staff.

The Contractor shall arrange and pay for the for the construction and
operation(including tables and chair and computers) of the field office and 1 lot
warehouse including provision for water, power, lighting;
The location and layout of the Contractor's facilities shall be subject to the
Project Manager's approval.   

The contractor shall provide temporary site office for the Project Manager
complete with Air-conditioning system, power, water, and internet connection.
The Contractor shall provide tables and chairs for at least seven (7) pax. The
contractor shall maintain the office facilities on regular basis and provide a full
time housekeeping personnel.

The Contractor may be required to relocate his sheds, workshops and offices


during the construction of the Works, and the costs of relocation is deemed to
be included in the Contract Sum.
Separate storage sheds for inflammable goods in an approved location must
be provided. No inflammable goods such as fuel oils, oil based paints,
kerosene, thinners, cellulose lacquers, bitumen or bitumen based products
etc. will be permitted to be stored in the building under construction.

The General Construction Works Contractor will provide the back bone for
the temporary water and power. Contractors who wish to avail of the
4 temporary power will have to provide their own electric and water 1 lot
meter,wiring,cabling, piping and pay for the electric consumption directly to
the General Contractor;

The General Contruction Works Contractor will provide general lighting for
the entire project. It is the responsibility of the trade and specialty contractors
4 to provide addequate and safe level of light to their respective work areas; 1 lot

The General Construction Works Contractor will provide generator sets for
use during MERALCO'S power outage. The respective contractor swill have
to provide their own generator sets in case the one provided by the Genral
5 Construction Works Contractror is not sufficient to service their needs. Lack included
of temporary power is not a sufficient ground for delays.

The Project Manager may instruct the Contractor to increase the amount of
temporary lighting if in the Project Managers opinion the lack of sufficient
5 included
lighting is affecting the quality of the installed works.

Water required for the testing of the Works will be for the Contractor's
5 account. included

Periods of non availability of water will not be grounds for additional payment
5 or extension of time. included

5 Temporary Plant and Equipment


The  Contractor shall provide, erect, maintain, move as required, and remove
upon completion of the Works, proper working platforms, scaffolding,
screens, safety nets, cranes, hoists, ladders, etc and all other plant and tools
included
necessary for the proper and orderly completion of the Works.

6 Bonds and Insurances

The Contractor shall obtain from an Insurance a surety bond equivalent to


6 the twenty (20%) down payment; 1 lot

The Contractor shall obtain a performance bond equivalent to the twenty


6 (20%) of the total project cost; 1 lot

The Bonds shall be extended at the contractor's expense if the Works are not
completed by the Date for Completion (as the same may be extended in 1 lot
accordance with the Conditions of Contract).

The ten (10%) percent retention money will be released after the Contractor
6 had presented a ten (10%) warantee bond equivalent to the ten (10) percent 1 lot
of the total project cost;

6 The Contractor shall obtain Contractor's All Risk Insurance Policy (CARI) 1 lot
The CARI shall be extended at the contractor's expense if the Works are not
completed by the Date for Completion (as the same may be extended in
accordance with the Conditions of Contract).

7 Sub-Contracting included
The Contractor shall not assign or sub-contract any part of the Works without
the prior written consent of the Project Manager and Owner.

8 Safety, Security and Housekeeping


The General Construction Works Contractor shall provide over-all security for
the Project site including Safety Officer. The respective trade and specialty
8 contractors shall provide their own security for their respective plant, offices 1 lot
and warehouses;

Subject to the General Construction Works Contractor conditions, trade and


8 specialty contractors can avail of first-aid facilities on the site; 1 lot

All Contractors  shall  report any and all accidents, injuries, or death to the
8 Project Manager immediately and furnish a full written report within three
days of the occurrence.

The Contractor shall be responsible for the safe storage and keeping of all
materials and equipment, delivered to the site, and shall replace at his own
8 expense any material and equipment that may be lost, stolen, or damaged by
whatever cause.

Contractor shall protect finished work from damage by any cause, and any
9 damage or loss arising shall be made good at his own expense.
The Project Manager has the right to suspend the Works in whole or in part if
9 compliance with the provisions of these sub-clauses are not observed, and
no extension of time or extra expense shall be allowed.

The General Construction Works Contractor shall provide sufficient


toilet facilities(portalet) around the Site. Other trade and specialty contractors
9 may use the provided sanitary facilties subject to the conditions to be
imposed by the General Construction Works Contractor.

On completion of the Works, the Contractor shall clean  up the Works
9  including all finished  surfaces and leave everything in a perfectly clean
working condition to  the entire satisfaction of the Project Manager.

If in the Project Managers opinion, the Contractor fails to provide proper bulk
bins or remove rubbish from the Site after reasonable notice in writing has
been given by the Project Manager or notice received from the relevant
9 authority the Owner reserves the right to employ another entity to remove
rubbish and deduct all costs and expenses incurred from any monies due or
to become due to the Contractor.

Burning of rubbish on site will not be permitted.

All Contractors shall provide for their Own adequate telecommunincations


facilities including internet at site;

10 Supervision included
The Contractor shall provide necessary competent supervisory staff for the
entire duration of the Works and for a time thereafter as required by the
Project Manager. The Contractor's senior staff must be proficient in both
written and oral English communication.

The Contractor shall provide sufficient numbers of QAQC, CAD Operator,


technical assistants, skilled, semi-skilled and unskilled workers as are
necessary for  the proper and timely execution of the Works.
The Project Manager reserves the right to require the Contractor to remove
from site, any person employed by him, who in the opinion of the Project
Manager misconducts himself or is incompetent or negligent in the proper
performance of his duties. Any person so removed shall not be re-employed
on the site in any capacity and shall be immediately replaced with a
competent person.

11 Coordination included
The Contractor shall coordinate with all Trade Contractors and Suppliers, or
other Contractors directly supervised by the PM, directly engaged by the
Owner and to afford them every facility to move about the site in the
legitimate pursuit of their respective and particular works and to provide them
the facilities required hereunder:

The trade and specialty contractors may be allowe subject to the conditions
by the General Construction Work Contractor free use of his scaffolding,
staging, ladders, hoisting facilities etc..,

The General Construction Works Contractor is required to maintain these


facilities longer than is necessary for his own use, nor to erect any special
scaffolding, staging etc., for use by others. Trade and specialty conractors
may have to provide scaffolds, etc for their own use to complete their
respective Works;

12 Protection of Works included


Any work carried out during inclement weather, of whatsoever degree,
shall be at the sole risk of the Contractor,  and  any  work damaged shall be
made good  at  his own  expense. The Contractor shall take all   necessary
precautions at his own expense, to protect the Works from  rain, typhoons,
monsoons and strong sun.

In the interest of safety of personnel and the Works,  the Project Manager
reserves the right to suspend the Works, if  in his opinion the situation so
warrants, and no extension of time or extra expense shall be allowed.

Trees should be preserved any relocation that may occur are to be endorsed
to the designer for treatment

13 Materials, Workmanship and Labor included


All materials to be installed must be new. Workmanship shall conform to or
be of a higher standard than the requirements of the Specifications and
13 the latest edition of the National Building Code. Materials and workmanship
shall be to the entire satisfaction of the Project Manager.

The Contractor shall furnish the Project Manager with all test certificates in
respect of materials and equipment showing compliance with the standards
required. He shall supply all warranties and/or guarantees for the materials,
13 equipment and workmanship. These warranties and guarantees shall be
handed over to the Project Manager;

Where  a  name of any proprietary article or any  firm  is  included in the


13 Specification and/or Bills of  Quantities,  it  is  to be read only as an
indication of the class  or quality of materials and workmanship required.

The Contractor shall observe the requirements of the Philippine Labor Code,


13 and any amendment  or  regulation issued by the Government or Municipality
during the Contract period.

The Contractor shall provide all necessary and  additional workers for day,
14 night, and overtime work to complete  the Works within the Contract Period.

14 Schedules included
The Project Manager will provide the Contractor the Master Construction
14 Schedule. The Contractor will then prepare their respective construction
schedule to fit in the requirement of Master Construction Schedule.

The Contractor shall submit his Construction Schedule with his bid. This
14 schedule shall show the manner and sequence of construction in bar chart
form and the critical path shall be identified on it.
The  Contractor  shall  carry  out  the  Works  in  strict  accordance  with  his
Bid and Master Schedules. If the Contractor is delayed in any area which
may affect the Date for Completion, then he shall notify the Project Manager
14 immediately giving the reason for the delay and the actions he proposes to
take to mitigate such delay.

Prior to the issuance of the Certificate of Practical Completion, the Contractor


shall issue for the approval of the Project Manager an as-built schedule

identifying the actual progress of the Works.

Submittal of the as-built schedule shall be a pre-requisite for interim


15 payments and payment of retention monies.

15 Reports included
The Contractor shall submit to the Project Manager each day throughout the
Contract Period a report showing a record of labor employed on the site
under each trade, materials delivered to the site, plant and equipment on site

and the weather throughout the day. The report shall be submitted to the
Project Manager by 10:00 AM the following day.

The format of the daily report shall be submitted to the Project Manger for
15 approval before commencement of work on site.
The Contractor shall submit to the Project Manager at the end of every week
15 a report on the general progress of the Works including details of:

- Progress for that week in terms of production rates


- Key decisions required from the Owners representatives in the next week
- Major milestones/ events for the next week
- Short term three week bar charts indicating progress in that week and
activities planned for the next two weeks.
This report is to be submitted before 10:00 AM every Monday.

The Contractor shall prepare and deliver to the Project Manager at the end of
15 every month his Progress Report. The Progress Report will set out:
- The true progress of the works
- Details of deviations from the approved Master Construction Schedule
- Circumstances adversely affecting work performance
- Particulars of remedial action taken or to be taken in respect of the
circumstances adversely
- Copy of the Master affecting Schedule
Construction the Workswith proposed schedule which
highlights proposed changes for the Project Manager's Approval
- Resources employed on and off site
- Major milestones/ events for the next month
At least three (3) copies of colored photographs showing the progress of
each major and significant area of the Works are to be supplied with the
Reports.

16 Practical Completion included

Practical Completion shall be in the opinion of the Project Manager when the


Works are complete to the stage where the Owner can have beneficial
occupancy of  the Works and where rectification of remaining defects shall
not cause undue disturbance to the occupants during the Defects Liability
Period.

Materials will be paid for at cost upon submission of actual invoices, plus a
certain percentage (%) of mark-up to be agreed upon with the Owner.

18 Quality included
The Contractor shall arrange for and carry out all tests required by the
Contract Documents, laws, regulations, Government and Municipalities at his
own expense. He shall inform the Project Manager seven days in advance of
carrying out such tests, so that the Project Manager can arrange for the
observance of such tests.

All test certificates shall be delivered to the Project Manager within three days
of testing. Failure of the Contractor to observe this time limitation shall mean,
the test or tests carried out will be repeated at the Contractor's own expense.

The Owner reserves the right to employ his own  testing agency to carry out
additional tests, and the Contractor shall afford such testing agency full and
unrestricted access to the Site and work to be inspected or tested, and is to
carry out any work of assistance as is reasonably required.

In  certain cases, the Project Manager may require a full size mock-up of
areas of work to determine the quality of the work and/or for the performance
of tests. Approved mock-ups shall be incorporated into the Works and will be
the standard required for the balance of that work.

19 Project Meetings included


Every week the Project Manager shall convene a meeting on site, and in
attendance shall be the Contractor's Project Manager and senior site
personnel and any others as required by the Project Manager. The Owner
reserves the right to attend at his option. The purpose of this meeting is to
discuss the progress of the Works and the resolution of any problems
hindering the progress.

Other matters shall be taken up as required. The  minutes of this meeting


shall be distributed by the Project Manager as soon as possible after the
 meeting date.

20 As-built Drawing
During the execution of the Works, the Contractor will ensure that he shall
keep a complete and accurate record of deviations from the Contract
Documents and shop drawings indicating the Works actually installed and
shall keep these records up to date and in a safe place. He shall submit one
sepia and three copies to the Project Manager at the time the deviations are 1 lot
recorded. CAD File to be submitted to the Owner for final submission

21 Operations and Maintenance Manuals included


The Contractor shall prepare and submit to the Project Manager all operation
and maintenance manuals required or directed by the Project Manager  and
the Specification two (2) months before the Works completion. The
Contractor shall submit five (5) hard boubd copies of every manual. The
Contractor shall arrange and pay for the teaching of the Owner's staff as
nominated by him, in the operation and maintenance as required. Time,
location, and duration of teaching shall be determined by the Project
Manager.

22 Testing and Commisioning


As a pre-requisite to  the issue of  the Certificate of Practical Completion, the
Contractor shall properly check, inspect and test the whole of the Works in
accordance with the specifications. All tests must be conducted in the
presence of the Project Manager and/or his representatives. The Contractor 1 lot
shall submit fully detailed test results and reports.
The Contractor shall be responsible for arranging and coordinating the
execution of all tests required including those integrated with the works of
Trade Contractors to ensure that the whole works conform to the
requirements of the Bid Documents. The satisfactory performance of all tests
required by the Contract Documents and submission of full test reports are
pre-requisites to the issue of the Certificate of Practical Completion for the
Completion of the Works by the Project Manager.

23 Project's Manager Site Hours included


The Owner's Representative(DCCD) attendance at site covers only from 8:00
A.M. to 5:00 P.M. Mondays to Fridays and 8:00 A.M. to 12:00 Noon
Saturday, but excluding holidays. Should the Contractor require the
representative to be in attendance outside of the times and days indicated
herein, then the Contractor shall submit his request at least twenty four (24)
hours prior, and pay the reasonable costs of the Representatives.

24 Others

The Contractor shall include cost of all items of work not shown on the
drawings nor called in the specifications but are required to complete the
Works.
