I. Pre-orientation: (Nurse’s and patient’s behavior before the NPI starts)

Nurse arrived at the hospital at 7:00 am. My patient was inside his room, wearing a
hospital gown. He avoids eye contact and has a flat affect. Appears distracted and focused on the
wall. No unusual movements observed.

II. Background Information: (Patient’s history of illness, include recent events.)

Patient diagnosed with Catatonic Schizophrenia

III. Setting of Interaction:

Inside the patient's room, the patient is sitting on the bed staring at the wall. The room is
well lit and has a few furnitures. The room was void of any visual or auditory distractions to
keep interaction as therapeutically conducive.

IV. Description of patient during interaction.

Appearance: wearing his hospital gown appropriately, hair slightly unkempt, facial hair
present and unshaved.
Behavior: Avoids eye contact, unusually still, only reacts with verbal stimuli, appears
nervous yet gets interested when talked about his friends
Affect: Flat
Pattern of Communication: Monotonous, direct, and short
V. A.) Objectives of the Nurse

At the end of ​ 8 ​ hours I will be able to:

1. Establish rapport and a trusting relationship with the client to be able to help the
patient to verbalize his feelings.
2. Establish boundaries between the nurse and client relationship
3. Use therapeutic skills in assessing and validating client’s perception and feelings
4. Facilitate the client’s expression of emotions

B.) Objectives of the nurse for the client:

At the end of interaction, the client will be able to:


1. Have rapport and a trusting relationship with the nurse and will be able to
verbalize feelings to the nurse
2. Establish boundaries between the nurse and client relationship
3. Be able to answer questions as precisely as possible
