Major Event Reporting

1 Purpose
The purpose of this management standard is to define the minimum requirements for reporting,
recording and investigating any major event arising during the course of Stork’s operations which does
not fall under the definition of an HSE incident. (For HSE incidents refer to Management Standard
ST00.653.1100 Incident Reporting, Recording and Investigation)

2 Scope
This management standard is applicable to all Stork businesses globally and shall be referenced when
developing regional and site level procedures to ensure compliance with the standard.

It also applies to those companies/joint ventures where Stork has operational control, and to those
companies/joint ventures that work for Stork as (sub) contractor and where Stork has operational
control. In case of doubt please contact the Stork group HSE team who shall provide support and
assistance in the decision process if a (sub) contractor is in scope or not.

This standard does not address requirements to comply with local legislation / regulations as these shall
be addressed in Regional Management Systems.

3 Minimum requirements

3.1 Major Event Reporting

All major events shall be reported immediately to the relevant manager.

All major events shall be assessed for actual severity based upon reputational damage to Stork and / or
business interruption.

The following are classified as major events:

• Written notice of violation / notice of non-compliance received from a regulator


• Written correspondence indicating penalties or fines assessed by a regulator
• Any regulatory inspection for which the inspector indicates that a violation was observed or
determined
• An environmental exceedance or deviation from an operating permit limit or parameter that
requires notification to a regulator
• All non HSE events that are reported to other Stork departments (e.g. HR, Legal, Finance), which
may include media interest in Stork or a claim relating to an alleged injury, illness or environmental
contamination

All major events relating to regulatory compliance shall be reported in English into the Regulatory
Compliance section of the Global Information Management System (GIMS) Reporting System via the Main
Menu > Workspaces > Project Tools > Regulatory Compliance.

All major events shall be assigned a severity rating (4 – 0) based on their actual severity. The required
notifications shall be based on the assigned severity.

Investigations shall be carried out as soon as reasonably practical following a major event. The level of
investigation required shall be determined by the severity of the major event.

Investigations shall include immediate, underlying and root causes.

Corrective and preventive actions stemming from investigations shall be tracked to completion. Action
